0277538604 2017-09-05 Drill / Deepen / Reopen No Violations During my inspection Mitchell Drilling Rig 1 was still on location. The well was equipped with 8.625 inch diameter casing with a casing head and 4.5 inch diameter casing with a cement head. On August 31st Formation Cementing cemented 3280 feet of 4.5 inch diameter casing within a 7.875 inch diameter borehole using 445 sacks of class A cement.
0448791744 2018-11-28 Preliminary Restoration No Violations On the day of my inspection I witnessed the well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, 1.5-inch diameter tubing and rods with a Sentry Pumping unit and an electric motor. Production equipment included a 210-barrel steel production fluid storage tank, horizontal separator and 2-inch diameter meter run with a .375-inch diameter orifice all enclosed within an earthen dike. All valves were open at the time of my inspection and the unit was pumping at the time of my inspection. It appeared that vegetation had been established around the location and preliminary restoration has passed according to Ohio Revised Code 1509.72 (A).
0586669525 2018-07-09 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the lease road and area surrounding the wellhead consisted of a gravel surface. There were no erosion issues. Vegetation has not yet been established. The well was equipped with 8.625 inch diameter surface casing with a casing head, 4.5 inch diameter production casing with a tubing head, 1.5 inch diameter tubing, rods, and a pumping unit with an electric motor. Production valves were in the closed position. The production equipment consisted of one 210 barrel tank, one horizontal separator, and a sales meter. The sales meter had a 2 inch diameter sales line, a .375 inch orifice plate, and a chart recorder. A contractor for Northwood Energy, Corp Oilfield Services, was on location burying the electric line to the pumping unit. Once the vegetation is established the location will meet the requirements for Preliminary Restorations written under the Ohio Revised Code 1509.072 (A).
1519932200 2018-02-27 Preliminary Restoration Violations Noted The purpose of my inspection was a status check on a preliminary restoration. During my inspection the pit used in drilling operations remained open. The pit was 90 feet long by 30 feet wide. The pit was lined and on the Northwest side of the pit 55 feet of liner had fallen below the fluid height. The pit was 1.5 feet from reaching total capacity. Within two months after the date upon which the drilling of a well is completed the owner or the owners agent, in accordance with the restoration plan filed under division (A)(10) of section 1509.06 of the Revised Code, shall fill all the pits. The pit remained open passed the duration allowed under 1509.072 (A). Within 15 days after receiving this notice the pit shall be closed.
1604107417 2018-03-08 Production Wells No Violations The purpose of my inspection was a follow up on a compliance notice. The notice was issued to Northwood Energy for failure to close the drilling pit 2 months after the well was drilled. I spoke with Corey McCudden with Northwood Energy on March 1, 2018 and he stated that he would get the pit properly closed within a week. During my inspection today on March 8, 2018 I found that the pit had been closed and the site was graded. Northwood Energy has taken the proper action to resolve the violation written on compliance notice numbered 1519932200.
1913300590 2017-08-28 Casing String Witnessed No Violations During my inspection Mitchell Drilling Rig was on location with a rotary rig. Mitchell Drilling had 100 feet of 11.75 inch diameter conductor in a 15 inch diameter borehole that was cemented and circulated to surface on 8/26/17. An 11 inch diameter borehole was drilled to 1354 feet; measurement from ground level ground level. I witnessed the rig tripping the last joint of drill pipe out of the hole and run 1330 feet of 8.625 inch diameter surface casing. The casing was new and weighed 23 pound per foot. Formation Cementing rigged up to the 8.625 inch diameter casing and pumped a pre-flush consisting of fresh water, gel, and lost circulation material. Following the pre-flush Formation pumped 330 sacks of class A cement with 2 percent gel and 86 barrels of fresh water for displacement. 15 barrels of cement returns were established at surface.
1962300470 2017-12-19 Completion Testing No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with 8.625 inch diameter surface casing with a casing head and 4.5 inch diameter production casing with a Frac valve in the closed position. Northwood Energy contacted me on November 3, 2017 and stated that they planned on hydraulically stimulating the well on November 7, 2017. All but one of the frac tanks have been removed from location. The pit used in the drilling process remains open and it is lined surrounded by a fence.
2029629485 2017-10-04 Completion Testing No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with 8.625 inch diameter surface casing with a casing head and 4.5 inch diameter production casing with a Frac valve in the closed position. The pit used in the drilling process remains open and it is lined surrounded by a fence. There are 20 Frac tanks on location and a water truck with the UIC number 376 was filling the last tank. I spoke with the water truck driver and he stated that the well was awaiting completion and all of the tanks have been filled.
