0438067539 2017-02-23 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges present read 470 pounds per square inch on the casing and 235 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment consists of a line heater, vertical separator, five sand traps, five gas production units, and eight 500 barrel tanks within containment. Identification was present and production records were current.

Enviro-spud ran 310 of 20 casing, and Calfrac cemented it in place with 558 sks of class A cement. I witnessed approximately 10 bbl of cement return to surface. The well passed the casing integrity test.
0924044291 2014-11-06 Preliminary Restoration No Violations My inspection today finds the site meets the preliminary restoration requirements. The tank battery is comprised of eight 500 barrel tanks set in a metal dike with a plastic liner. Four 500 barrel temporary tanks are set on site in a plastic liner. A liquids pump system is set up at the back of the production site along with a large compressor. Vegetation has been established around the site.

1408352869 2014-11-14 Production Wells No Violations I am onsite in response to the explosion on 11/12/14. Most of the liquids discharged at the liquids pump system were consumed in the explosion and ensuing fire. A ten by ten area of limestone has a coating of condensate. Blue Racer Midstream General Manager Larry Palmer was notified to clean the affected area.
1415855682 2014-11-12 Production Wells No Violations I am onsite in response to an explosion at the production area of the well pad. The source of the explosion is believed to be a liquids pump skid owned and operated by Blue Racer Midstream.
