0294944187 2016-01-13 Production Wells No Violations At 11:42 am, I received a telephone call from Jamie Kominich indicating that a small glycol release occurred on the NBL 33 well pad sometime between 6:00 and 9:00 am. I arrived on location at 12:23 pm. The site was covered in three inches of snow. I inspected the site and found an area approximately 12 by 15 with the snow melted from the surface. Around the melted area a 3 wide band of snow was discolored pink by the atomized mist. I spoke to Cody Craker a CNX employee and he stated that the cause of the release was human error. Mr. Craker indicated that T-K personnel set the Kimray T-12 temperature control regulator too high, causing the over pressurization of the production unit heating vessel, resulting in the release. The estimated total of material (50/50 glycol/water) lost is three to five gallons, with all material remaining on site. I instructed the company to remove all contaminated soil and properly dispose of the material. The company will turn over a copy of the waste manifest as soon as possible. Work on the site is estimated to be completed by the end of the day.

The gas line ruptured in the steel to plastic transition. The fuel line is no longer needed and will not be replaced.
0566305987 2017-03-09 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. Gauges present read 400 pounds per square inch on the casing and 200 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment consisted of three sand traps, three gas production units, six 400 barrel tanks, and one combustor. Identification was present and production records were current.
0775732539 2015-06-16 Production Wells No Violations A fire was discovered at the NBL 33 well pad by U.S.A. Compression personnel. The well pad was shut in at the time of the discovery due to high line pressure. Upon finding the fire, the Emergency Shut Down was activated; however, the fire did not stop. The Caldwell Volunteer Fire Dept. was called at 3:00 pm and arrived onsite at 3:16 pm. Fire Chief Reed stated that a fire was emanating from under the ground. The Fire Dept. sprayed the low pressure separator adjacent to the fire to keep it cool, until Bill Palmer with TK Services shut in the fuel gas line and the Vapor Recovery Unit line. Once those valves were closed the fire went out. I arrived on scene at 4:22 pm and the fire was out.

My inspection of the area finds no visible signs of contamination from liquid hydrocarbons. The fire occurred between a low pressure separator and a glycol line heater. The hole in the ground where the fire was burning is directly adjacent to the fuel gas line and is a likely source.

The production equipment consisted of six 400 barrel tanks, three sand traps, three gas production units, and one combustor. The natural gas pipeline tie in was located on the south side of the pad. The identification was posted by the wellhead.
