0573498122 2019-03-06 Production Wells No Violations On the day of my inspection I witnessed production equipment included a 210-barrel steel production fluid storage tank, 100-barrel steel production fluid storage tank, horizontal separator, drip and 2-inch diameter meter run with chart recorder and current chart, all contained within an earthen dike. There was a small leak on the dump line below the dump valve and approximately 1 gallon of crude oil on the ground. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, 2.375-inch diameter tubing with rods and an Alten pumping unit with electric motor. There was a dike built around the well head and it appeared that Peatsorb had been applied for several years. The preliminary restoration was passed in 2013 by ODNR inspector Eric Conrad, it is still showing up on activities list. I contacted Corey McCudden with Northwood energy and informed him of the problems that I witnessed. All valves were open at the time of my inspection, Identification was present on the production equipment, and production was reported for 2017.
0798930087 2012-08-29 Production Wells No Violations My inspection today finds Producers Service Corp. fracturing the well. I observed and gathered information about the frac. The well is perforated from 1611 to 1936 with 84 shots in the Ohio Shale. The formation broke down at 1717psi. A total volume of 333668 gallons of H2O were used, at an average pressure of 2053psi and an average rate of 43.6 bbl per minute.
0966436058 2013-05-23 Production Wells No Violations The well is being produced by a pump jack. The tank battery consists of one 210 bbl tank, one 100 bbl tank, a separator, and a meter. Vegetation has been established.
1413898539 2016-05-11 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery consists of eight 400 barrel tanks set inside of a plastic line steel containment system. On the opposite side of the pad there are eight production units, also set in containment. Several pieces of construction equipment are parked on the location and are being used for the reclamation of the adjacent fresh water impoundment.
