0105222550 2013-02-14 Production Wells No Violations My inspection today finds that the pressure relief valve has been opening and has released approximately 2 bbl of oil. I contacted Northwood Energy and Corps Oil Field Services and told them of the spill. Within two hours there were five contractors on site to clean the spill. By 3:00pm. the spill had been cleaned.
0160763153 2019-04-23 Production Wells No Violations The purpose of my inspection is to follow up on a previously issued compliance notice. On the day of my inspection I witnessed production equipment including a 210 barrel steel production fluid storage tank, 100-barrel steel production fluid storage tank, a horizontal separator which appears to be the primary means of separation and a vertical separator that is acting as a fluid drop out for the sales line, there is also a drip and 2-inch diameter meter run with .500 orifice. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing with 2.375-inch diameter tubing, with rods, a lubricator, and a Churchill Pumping Unit with an electric motor. The lubricator on the well head was running into a 50-gallon steel drum. At the time of my inspection it appeared that the contaminated soil had been removed from around the well head, the earthen containment was still in place but appeared to be free of pollution and contamination. At the time of my inspection Mike Corp a contractor for Northwood Energy Corporation stopped by, I informed Mike that the trash and jugs around the pumping unit needed to be cleaned up, but compliance issued for pollution and contamination will be closed.
0196480471 2015-11-09 Production Wells Violations Noted My inspection today finds an area of crude oil contamination 45 from the wellhead. An area approximately 40 square feet is discolored and has a strong odor of crude oil. A small earthen dike around the wellhead has a couple inches of oil contaminated absorbent lying in it. The tank battery has multiple leaking pipe connections with some of them allowing natural gas to escape to atmosphere and oil to reach the ground. Remove all contaminated soil and repair

0452842061 2013-02-14 Production Wells No Violations Notification was received at 7:35 Am by Eric Byers of Byers Oil & Gas that He observed the well storage tank battery separator dispersing fluid. I made notification to the lead inspector Eric Conrad and proceeded to respond to the site. Upon arrival @ 7:40 Am it was observed that the separator check valve was releasing oil into the tank battery. At 7:45 Am a Northwood representative arrived and shut the well in stopping the dispersant of product. I assisted Inspector Conrad with the spill report and left location at 9:00 Am. Notification was made to the Southeast supervisor Rocky King at 7:38 Am and a copy of the spill report sent Kyle Tharp and Rocky King.

The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing with 2.375-inch diameter tubing, with rods, a lubricator, and a Churchill Pumping Unit with an electric motor. There was approximately 80 square feet of pollution and contamination in a dike that was created around the well head. There were multiple areas on the well equipment that appeared to be leaking, and the lubricator valve was open and running to a 50-
1100097963 2012-09-11 Production Wells No Violations My inspection today finds the well is equipped with 4.5 casing, 2.375 tubing, rods, and a pump jack. The tank battery consists of one 210 bbl tank, one 100 bbl tank, a separator, and a meter inside of a dike with proper identification. The location has recently been seeded. Pipe line work is under way.
