0184172773 2017-07-10 Production Wells No Violations The purpose of my inspection was a follow up on a compliance notice numbered 1477077738. The compliance notice stated that the well was in violation of 1501:9-9-05(A)(1) Well insufficiently equipped to prevent escape of oil and gas, 1501:9-9-05(A)(10) Failure to legibly identify, and 1501:9-9-05(A)(2) Violation of tank spacing requirements. During my inspection the well was equipped with a 4.5 inch diameter production casing with a casing head, 2 .875 inch diameter intermediate casing with a tubing head, and 1.5 inch diameter tubing with 2 inch diameter production lines attached. Production valves were in the open position. The production equipment consisted of one 210 barrel tank and one 1550 gallon poly tank. Identification was located on the 1550 gallon poly tank. No leaks were observed from the wellhead, the 1550 gallon poly tank had been moved to meet setback requirements, and the identification was located on the 1550 gallon poly tank. All violations from the compliance notice have been resolved.

0825898092 2019-01-04 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, and 1.5-inch diameter tubing, that appeared to be open flowing, there appeared to be a domestic gas hookup attached to the well head, but the valve was closed at the time of my inspection. There was a 37-barrel plastic production fluid storage tank, and a 210-steel production fluid storage tank that were near the well head and no longer in use. Production equipment included a 100-barrel steel production fluid storage tank, horizontal separator, 2-inch diameter meter run, with drip all contained within an earthen dike. The production equipment was located off township road 128 at State Farm Road, near Caldwell lake. At the time of my inspection all production valves were closed and well appeared to be idle, identification was present on the production equipment, and production was reported for 2017.

1477077738 2016-10-21 Production Wells Violations Noted On the inspection today I was accompanied by Supervisor Eric Conrad. Eric received an anonymous complainant about an H2S leak on this well (complaint #5458). We arrived on location and could hear that gas was leaking from the wellhead. After closer inspection we found that neither of our gas detection meters could read any H2S and, no H2S could be smelled in the area. The well is equipped with a 4 1/2 inch diameter wellhead, 2 7/8 inch diameter intermediate casing, 1 1/2 inch diameter production tubing, and 2 inch diameter steel production lines. This well is located close to two water ways and a road. Two valves on the wellhead have no plugs in them. The well had a 1600 gallon plastic tank about 10 feet from the well head and there was a 210 barrel steel tank about 150-200 feet away from the well head. The plastic tank does not meet the set back requirements from tank spacing according to OAC 1501:9-9-05(A)(2) this tank must be 50 feet away from the well head. The tanks had no sign of proper ID on them and require ID. The company must post proper identification in accordance with 1501:9-9-05(A)(10). The well must be repaired to prevent the escape of gas, plugs put on all valves, and proper identification posted within 15 days of this compliance being issues.

equipped with a 4.5 inch diameter production casing, 2 .875 inch diameter intermediate casing, 1.5 inch diameter tubing, and 2 inch diameter steel production lines. The well was not leaking and plugs have been placed in the valves. Well insufficiently equipped to prevent escape of oil and gas violation is now satisfied. The production equipment consists of one 210 barrel tank and one 1600 gallon poly tank. There was not any identification located on the tanks or the wellhead. Identification must be posted to bring the well back into compliance.
