0266653851 2016-11-29 Production Wells No Violations The purpose of my inspection today is a status check on a producing well. during my inspection the well was equipped with 4.5 inch diameter production casing, 1.5 inch diameter tubing, and a plunger lift system. The valve on the tubing and the valve on the production casing is open. There is a 1 inch steel domestic gas line, attached to the production casing, with a Little Joe regulator, and an open valve. The production equipment for this well consists of two 100 barrel tanks, and a horizontal separator. Production equipment is within an earthen dike and the well identification is located on the 100 barrel tank.

1490965474 2017-03-31 Production Wells No Violations The purpose of my inspection was a status check. Northwood Energy has recently installed a pumping unit and removed the plunger lift system. During my inspection the well was equipped with 4.5 inch diameter production casing, 1.5 inch diameter tubing, rods, and a pumping unit powered with a natural gas motor. All valves were in the open position and the unit was pumping during inspection. The production equipment consisted of two 100 barrel tanks, one drip, one horizontal separator, and one chart meter. All equipment was within an earthen dike. The identification was located on the 100 barrel tank, the production records are current, and there were no violations present.
1557248477 2019-05-06 Production Wells No Violations The purpose of my inspection was to follow up on a previously issued compliance notice. On the day of my inspection I witnessed production equipment included two 100- barrel steel production fluid storage tanks, horizontal separator, drip and 2-inch diameter meter run with chart recorder and current chart. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, 1.5-inch diameter tubing with rods, a Churchill pumping unit with natural gas motor. At the time of my inspection the 8.625-inch diameter casing head had a small leak, there was still crude oil residue inside of the earthen containment, and the earthen containment was full of water. It appeared that the water had reached a level that it was starting to overflow out of containment. I will contact Bill Arnholt and inform them there are still issues that need to be addressed.

1829658677 2019-04-12 Production Wells Violations Noted The purpose of my inspection was a status check, when I arrived at the location, I noticed that there had been recent excavation of dirt around the well and tank battery of approximately 1000 square feet. Shortly after I arrived on location Ron Corp with Corp Oil Field Service and operator of the well arrived on location. I inquired to him about what had happened, and he informed me that they had a tank leak and crude oil had made it outside of the earthen containment. I informed Ron that if there was an incident like this that it needed to be reported to me. The production equipment consisted of two 100-barrel steel production fluid storage tank, horizontal separator, drip and 2-inch diameter meter run. The tank on the east side of the production area was damaged at the top and it appeared fluid had run out, there were other possible damaged areas on the bottom but could not be confirmed with oil and water inside of the earthen dike. The well was equipped with 4.5-inch diameter production casing, 1.5-inch diameter tubing, rods and a pumping unit with natural gas motor. Identification was located on the production equipment and production was reported for 2017. I attempted to contact Mike Corp and determine when the incident occurred and am currently waiting on a phone call from Corey McCudden with Northwood Energy to discuss problems in the area. Photographs were taken to assist with the inspection and a compliance notice will be issued for pollution and contamination and failure to notify according to OAC 1501:9-8-2 and 1501:9-8-2-(A)(4).
