0242660772 2016-02-22 Production Wells No Violations My inspection today finds the well is equipped with 4.5 casing and 2 tubing with rods and a pumping unit. All valves are closed on the wellhead with exception of one 1 valve that may feed domestic gas to a nearby home. I will research further to determine if the well is used for domestic gas. The tank battery consists of one 210 barrel tank and a production unit. This well is part of a group of wells that Mark Warehime, production manager for Atlas, told me were shut in due not no market for gas.
0669166068 2017-12-19 Production Wells No Violations The purpose of my inspection was to follow up on compliance notice 1341410168. During my inspection the well was equipped with 8.625 inch diameter surface casing with a casing head, 4.5 inch diameter production casing with a tubing head, tubing, rods, and a pumping unit. The commercial production valves were in the closed position. A domestic gas line was attached to the 4.5 inch diameter production casing with an open valve and a regulator. The packing on the casing head was slightly leaking natural gas. The production equipment consisted of one 210 barrel tank and one horizontal separator. The domestic gas production brings the well back within compliance. A failed effort was made to speak with the landowner to discuss the domestic use, the resident was not home.

A small gas leak can be heard at the packing of the 8.625 by 4.5 casing wellhead. The tank battery consists of one 210 barrel tank and a production unit.
1094206883 2012-07-03 Production Wells No Violations FOLLOW UP TO NOV # 1341410168 Upon receiving request from landowner to inspect site for compliance found well not to be producing in commercial quantities and last production reported on said well was back in 2005 all valves at wellhead were shut on the day of my inspection and it was noted that there is a domestic hookup to well but valve was shut at the time of my inspection, took photos of well and tank site.

1182429940 2014-07-03 Administrative Inspection No Violations I called and spoke to Mark Warehime Field Supervisor for Atlas Noble LLC at 7:22 AM. Mr. Warehime stated that the well has not been approved for plugging yet.
1265576833 2018-12-27 Production Wells No Violations On the day of my inspection I witnessed production equipment including a 210-barrel steel production fluid storage tank, horizontal separator, and drip. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, 2.375-inch diameter tubing, with rods and a pumping unit with an electric motor, the electric motor was no longer hooked up. There appeared to be a domestic gas hookup that the valves were open. There was trash and debris piled near the well equipment. There were no identification tags present and production has been reported since 2006.
1340017555 2012-06-18 Production Wells No Violations Inspection of the well head found the following equipment 8.625 surface casing -4.5 production casing -2.375 tubing - rods& pumping unit ,with the commercial valves closed and a domestic hook up present with the domestic valve open and no reported commercial production since 2005. A single 210 barrel production tank & separator are present with proper well identification .Inspector will note that the tank dike is insufficient and sits next to a stream and should be brought to SPCC standards before being put back into production . Idle well and will refer to lead inspector Dave Ball for corrective action.

During my inspection the well was equipped with 8.625 inch diameter surface casing with a casing head, 4.5 inch diameter production casing with a tubing head, tubing, rods, and a pumping unit. The commercial production valves were in the closed position. A domestic gas line was attached to the 4.5 inch diameter production casing with an open valve and a regulator. The packing on the casing head was leaking. The production equipment consisted of one 210 barrel tank placed by a nearby stream and one horizontal separator. Original violations still exist. Pictures were taken to assist this inspection.

The production valves were in the closed position. A domestic gas line was attached to the 4.5 inch diameter production casing with an open valve and a regulator. The production equipment consisted of one 210 barrel tank placed by a nearby stream and one horizontal separator. No dike was in place leaving the original violation of 1501:9-9-05 A (9) still in existence.
