0198270213 2017-09-21 Plug / Plug Back No Violations On the day of my inspection I accompanied ODNR Inspector Austin Gutridge. Please see his comments for a detailed report. Chipco Oil and Gas Well Service had a service rig, cement truck, a bulk cement truck, and a pump truck on location to plug this well. The well was produced from the Clinton Formation with a total depth of 6,036 feet, 8.625 inch diameter surface casing set at 1473 feet, and 4.5 inch diameter production casing set at 6014 feet. The well was producing through perforations in the 4.5 inch diameter production casing from 5946 feet to 5884 feet in depth. Chipco Oil and Gas had 1.5 inch diameter tubing at 6014 feet in depth and set a 400 foot plug consisting of 30 sacks of Class A cement for the bottom plug to cover the Clinton. The 1.5 inch tubing was pulled up to 5110 feet and gas started to flow from the wellbore insinuating that the perforations took a portion of the first plug. So an additional isolation plug was set from 5110 feet to 4844 feet in depth with 20 sacks of Class A cement. The 1.5 inch diameter tubing was tripped out of hole.

All equipment and debris have been removed and the location had been graded. Vegetation has not been established. Once the vegetation is established location will meet the requirements for 1509.072 (B).

1483028958 2016-12-29 Production Wells Violations Noted The purpose of my inspection today is a status check on a producing well. During my inspection the well was equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing, 1.5 inch diameter tubing, and a plunger lift system. The 4.5 inch production casing has a domestic gas line attached with a meter and an open valve. The 4.5 inch casing has a repair clamp right above the 8.625 inch casing head. There is a gas leak right below the repair clamp. The gas is escaping from either the repair clamp on the 4.5 inch casing or from the 8.625 inch casing head. The well shall be equipped so that no gas is allowed to escape into the atmosphere with the exception of gas flares as per 1501:9-9-05 (B). I met in person with the well tender for Enervest, Dave Williams, and I explained the violation and that it needs to be fixed.

1506345064 2017-09-25 Plug / Plug Back No Violations During my inspection the surface plug was completed cured and solid, 3 foot from surface grade. Chipco Oil and Gas Services was rigging down on location and the restoration plan was discussed. Chipco is going to dig down 30 inches around the surface casing, cut off the surface casing, install the identification plate, and restore the location.
1890746953 2017-09-22 Plug / Plug Back No Violations During my inspection I witnessed the 4.5 inch casing pulled up to 1573 feet and the bottom of the surface casing plug was set from 1573 feet to 1373 feet with 60 sacks of Class A cement.

During my inspection the location had been restored and the production equipment had been removed. Location will still need to be seeded to establish vegetation. A follow up inspection will be completed.
