0490783216 2014-09-23 Plug / Plug Back No Violations 8:30 AM - Crew and I arrived on location. They had brought wellhead with a collar that they thought could be installed but threads not matching up.

11:30 AM - Welding truck arrived to weld the collar/wellhead on the existing 5.25 diameter casing. 12:30 PM - 2:30 PM - Tubing and Cemenco crew arrived. 3:15 PM - 4:45 PM - I left location. 5:00 PM - Rig crew and Cemenco left location, after washing up.

0513783599 2014-09-24 Plug / Plug Back No Violations 8:00 AM - Rig crew arrived on location.

8:30 AM - I arrived on location. The crew excavated around the casing, cut off casing 30 below ground level and adding 4 sacks of dry cement to bring the cement top to surface. The steel plate with the permit number was welded onto the casing, the hole backfilled and the ground graded. The small amount of contaminated soil between the creek and the hole was dug up with shovels and loaded into a container to be disposed. The disturbed ground was seeded with a pasture mix seed. 10:30 AM - Rig and other equipment moved to next location. All left location.

0701799510 2014-01-16 Orphan No Violations My inspection today with Cindy Van Dyke finds the well is equipped with 5 3/16 casing reduced to 2 pipe. Oil is leaking from the end of the 2, though not at the time of inspection. A 6 x 3 area of soil is saturated along the bank of a small stream. A slight sheen of oil is visible on the stream. Oil absorbent pads will put in place next week.
1389989472 2014-01-16 Orphan No Violations My inspection this day with Inspector Eric Conrad is to research this orphan well and enter it into the Orphan Well Program. The visit is the result of a complaint from landowner Aaron Reed, and today found the well dripping oil onto the ground at the edge of Otter Run. The well is also releasing a small amount of gas (tested 5 ppm. LEL at end of 2 steel line). The well consists of 5 3/16 OD casing swadged down to 2 3/8 OD tubing which turns and runs parallels to the ground for 6 to the edge of the creek. The casing stands 5 above ground level and is 5 from the edge of the creek. There is an oil-saturated spot 6 x 3 running along the creek where the oil drips out of the steel line and there is a slight sheen on the creek at that location. The creek is @ 3 wide and 3 below ground level. Access to the well is off St. Rt. 285 turning right at the mail box reading 45360, then traveling between the barn and the electric pole on the west side of the creek to the well. Pads will be placed next week to absorb the oil on the creek and the landowner has agreed to place a plug and valve to stop the leak until it can be plugged. SEM is 237, pictures taken.
1390504718 2014-01-21 Orphan No Violations My inspection this day was to show the well to orphan well plugging contractor Ron Earley, of D & L Drilling. We found the well still releasing a fair amount of gas out the 2 line running perpendicular to the casing. We removed this 2 line at the elbow and could look down into the hole, finding that the hole was full of oil which bubbled out and over the top of the hole. To temporarily stop the leak, we put a plug and valve on top of the 2 nipple and just cracked the valve so that pressure would not build up in the hole. This preventive measure should stop the well from leaking into the creek until it can be plugged completely. Well records show the well drilled in 1942 to a total depth of 178 and called dry. 1422040637 2015-01-23 Final Restoration No Violations My inspection this day was to confirm reclamation has been completed. All pluggging and well related equipment had been removed. I found the final grade satisfactory and good vegetative growth over the entire well location.
1441659402 2013-01-03 Orphan No Violations My inspection today finds the well is equipped with 5.5 casing. The casing is reduced to 2 pipe. There are no valves on the well. The land owner stated that the well periodically leaks oil into the nearby stream. There is a 4 oil kill on the bank of the creek.
