0989247273 2014-11-05 Preliminary Restoration Violations Noted The landowner Gary Okey called on 11-3-14 on vegetation issues at well site. The complaint # is 4851. The site was worked on this summer due to same issues. The company worked on areas with not much progress happening. North of the well head is areas ranging from approximately 8x32. The largest being from 40x100. There are no errosion issues but there is no vegetation due to the chlorides in the soil. The highest field sample was 400 parts per million of Chlorides. Remediate according to the Best Managemnt Practices and establish vegetation on the affected areas.
1226576122 2015-06-22 Production Wells No Violations The site at the well has been vegetated back and the issue has been resolved on 6-22-15 by Patrick Shreve. This resolves the compliance notice # 989247273 and the complaint # 4851.
1248389107 2014-06-19 Production Wells No Violations The landowner Gary Okey requested the inspection of the well site because there was no vegetation established at the site after the well was drilled. I did not have Quantabs to perform a field test. I did take pictures of the affected area on the day of 6-19-14. I will contact company after verifying my findings.
