1481325720 2016-12-09 Production Wells No Violations At 15:23 I received a call from New Concord Fire Chief Brent Gates stating that land owner Randy Bridwell was smelling gas near his property on Clay Pike. I called Mr. Bridwell and he stated that a line was leaking gas near the well on his property. Upon arrival at 16:00 following up on Complaint ID 5491, I inspected the well finding it equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 1.5 inch diameter tubing, rods, a compressor unit, domestic gas service and a Wildcatter 57 pumping unit powered by electricity with the belts burnt off and the power in the auto position. One of two valves was open on the tubing and 2 of 2 valves were open on the casing, 1 supplying the domestic gas service and 1 was open to the atmosphere. The 1 open to the atmosphere previously went to the compressor unit but at this time was separated and leaking gas. Readings were taken with ODNR Honeywell PhD6 meter revealing greater than 10 LEL gas present at the time of inspection. I made numerous unsuccessful attempts to reach Petrox the owner of record. The production equipment included 2-100 barrel tanks, a 30 barrel tank, a horizontal separator, a 2 inch salesmeter with a.25 inch orifice and a chart recorder without a chart all within an earthen dike. The east 100 barrel tank has a small leak in the burner tube and the 30 barrel tank needs a 2 inch plug. Identification was present and present and production records were current. ODNR Muskingum County Inspector Pat Shreve arrived and again with no response from the company the valve was closed on the casing which was open to the atmosphere. Inspector Shreve will continue to try to contact Petrox to arrange a meeting tomorrow to address the issues.
1481820395 2016-12-13 Production Wells No Violations Jimmy Wiles and I inspector Patrick Shreve stopped by the well to meet with Bill McGraw of Petrox INC. and his helper. We all walked into the well site and I stated to Bill that there can be no more venting of the gas into the atmosphere thru the Kim- Ray pop-off devices. The wells that had the pop-off devices were on the pumping unit supplied wells.

-Ray pop-off and the opposite side was opened to domestic gas. The well apparently loaded up with fluid due to not being able to pump the fluid. The gas blew the pop-off open and dumped the gas out of the well. The landowner then called in due to no gas pressure. Doug and I went in and shut the valve on the tubing annulus then shut the power off. Then Doug and I went to the tank site. There were 2-100 barrel steel tanks and 1-30 barrel steel tank and a horizontal separator. There was leaking burner tube on the one tank battery. Doug Bennett had a surface job to go get prepared for so I stayed and called Bill McGraw of Petrox. He called back at 6:11 and gave me a number to the pumper Chad Lowery at 740-607-0652. I called Chad and he had a helper come out named Jorden Lowery to the site. The both of us went into the site and went thru everything. The automation set up was fixed so not to create an issue. The pumping unit was not hooked to the compressor at the site. He would install new belts on the pumping unit. The company will be moving the 1-100 barrel steel tank down by the well. There will be a new compressor installed and the gas will be compressed and sold into Atlas line. The work should start by next week. The Kim- Ray pop-offs will be removed from the wellheads. The company will call when work commences. There
