The compliance notice stated to have the well plugged within 90 days. It has now been 90 days since the compliance notice was issued and there has not been any action taken to gain compliance. The 8.625 inch surface casing and the 4.5 inch production casing is still opened with no bull plug or valve to prevent gas from escaping into the atmosphere. Pictures were taken to assist with this inspection. During my inspection I was accompanied by ODNR inspector Pat Shreve.

1460655252 2016-04-14 Production Wells Violations Noted The purpose of my inspection today is a Civil Action Follow---10 5. During the time of my inspection the production equipment includes one 210 bbl. tank and one horizontal separator. Equipment is over grown by vegetation. The well is equipped with 8.625 inch surface casing and 4.5 inch production casing. The casing head has a threaded port that is opened with no plug, line, or valve. That leaves the 8.625 surface casing opened with no way to prevent gas from escaping. Above the surface casing head the 4.5 inch casing rusted in half leaving that 4.5 inch casing open ended. The rusted off portion of the 4.5 inch casing is now lying on the ground beside the well; completely disconnected. That leaves the 4.5 inch production casing completely opened with no possible way of shutting in to prevent gas from escaping. The well owner/operator shall submit a written plugging schedule to Austin Gutridge at 5880 Memory Rd., Zanesville Ohio 43701 within 15 days, per OAC 1501:9-11-05(B), and plug the well within 90 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
