0060517548 2016-06-21 Plug / Plug Back No Violations Scott and I met with Eclipse on permit number 119-22571. They had reclaimed the site. They ran fertilizer and seed. Then they mulched the entire area with straw. Bruce Carpenter and Tony Blizzard were at the site. I will check back at a later date.

1432360074 2016-05-26 Plug / Plug Back No Violations The purpose of my inspection is a Final Restoration. Bruce Carpenter and Tony Blizzard met me on site. The restoration looks good with some areas that will need smoothed out with some discs and reseeded and mulched. There are no erosion issues at the site. The landowner will take care of the rest so I suggested a Landowner waiver. They will get with the landowner and get one completed.
1441033346 2015-08-27 Plug / Plug Back No Violations Wright well service has rig over well. They pulled all 2.375tubing out and bullheaded a cement plug with Formation Cementing at a depth of 4090-3740 thru the 5.5casing at 15.50/lb.
1447768180 2015-11-17 Plug / Plug Back No Violations Dozer was moved on site. Per Tim Altier there will be Monroe Drilling moving in to clean cement out and to replug the well. Methane is coming in the well from hole in conductor somewhere.
1453403937 2016-01-20 Plug / Plug Back No Violations 119-22571 well owned by Eclipse Resources-Ohio Inc... On day of inspection I met with Rocky King and Tim Altier of Eclipse. The well has been plugged since 9-1-15. The well was being monitored because methane gas had been coming from the coal zone in the well. Eclipse had tested the gas and stated that the gas was methane coming from the coal. The cement top was at 15 from surface. Originally the company that drilled the well had run 190 of 10.75 conductor. This would put casing thru the coal seams. The 8.625 casing was pulled from the seat at 715 after drilling the well. During the plugging process there was a plug set from 100 in and 100 outside of the 7 casing. There was only 715 of 7 casing pulled during the plugging. There was cave in on this well. During the plugging tubing was 2.375 and the plug were placed across the Big Injun from 645-445. Then tubing was pulled up to 350 and set plug back to surface. After the top plug settled back to 15, the well had been working gas out thru a hole near the coal seam in the conductor. From the time the well was plugged until now the well has become static. The decision was made to top the well off with grout and reclaim back in 2-3 months.

Then they ran 2.375upset tubing to 645 and set a 200plug at the Big Injun. They then pulled tubing up to 350and set a surface plug in the 10.75casing and the 10open hole. Formation Cementing performed the cementing job. All plugs circulated.
1724188071 2016-03-24 Plug / Plug Back No Violations The well was plugged and I was looking at the restoration on the lease road and site. There are sufficient water bars in place and there has been hay placed over the site. There is a large portion of the lease road not seed or mulched. I called Tim Altier on the issue. He will get with Tony Blizzard and get fixed.
1763958491 2015-08-31 Plug / Plug Back No Violations Wrights rig over the well. Tagged cement plug at 3727. Ripped 5.5casing off at 2784. Then pulled 2- 31joints out and set Big Lime plug at 2726-2426. Formation Cementing ran 54 sacks of Regular Class A cement. Then Wright pulled 5.5casing up to 1322-1122 and set a 200 at the bottom of the 7seat. Set cement plug of 100inside and 100outside at the bottom of the 7casing. This is the berea plug. The drilling rig pulled the 8.625casing after drilling the well. The had installed 60 sacks of clay and 7 sacks of gel. The 8.625seat was at 715. Wright could not get stretch or telegraph until 505. Then wright ripped 7off at 715. There was clay caving in on the backside of the 7casing causing the pipe to be ripped at 715. Wright pulled all 7out.
