0131564272 2013-11-20 Production Wells No Violations Inspected the Kunze #1 (#3947) as a standard compliance agreement follow up to the original Notice of Violation (#1209649606) written by Inspector Sam Faust 5/1/2008. No change in status from previous inspection: non-compliant. A plug permit was issued 5/27/2008 and expired 5/27/2009. Inspector Tom Benko performed a follow up inspection on 1/11/12 and noted no change in status and contacted the operator who stated he was working on ownership transfer and/or possible temporary inactive status. Subsequent inspections have shown no change in status, no legible well identification or emergency contact information posted, the well is incapable of production (no pumpjack) and no production has been reported with the State of Ohio since 2000. The Kunze #1 (#3947) shall be plugged, placed back into production, placed into temporary inactive status (if applicable) and correctly identified. Status: sent operator corrective action inspections. Will continue to monitor.
