M&R Investments ran a cement bond log on the well at time of purchase. A bridge plug was set at 260 and cement pumped to attempt to seal the hole that was reported to be below 200 feet from grade. Upon drill out of cement the hole was not sealed. 6/5/2012: operator was given approval to install and cement a 2-7/8 inch casing then install 1.5 inch tubing on packer for injection. 7/8/2013: it was determined that the hole in the 4.5 inch casing was between 62 and 90 feet deep. Frac sand was installed over the perforations to a depth of 2960 feet. The 2-7/8 inch liner was set at 2958.2 ft. and cement to the surface. 7/15/13: begin drilling out cement and wash out frac sand. 7/26/13: Set Parmco tension packer and 1.5 inch tubing. Circulate water and inhibitor, pressure test and pressure held for 15 minutes at 320 psi.

He reported the fittings on the wellhead have been replaced and updated, the annulus was pressure up. Fishburn was scheduled to clean out the 210 barrel replacement storage tank. A crane is scheduled to lift the old tank out of the containment area to be scrapped and the replacement tank will be installed into the containment. The operator was advised the well annulus and the flow line will need to be tested before the well is put into production.

The service company used an Halliburton R-4 packer to attempt to locate the hole in the 4.5 inch casing. Repeated testing at subsequently shallower depths the hole in the casing was determined to be between 62 and 90 feet below grade. See 12/12/2011 report for field activities previously conducted.

1504875037 2017-09-07 Salt Water Injection Well No Violations The well was reworked in July of 2013 and passed the mechanical integrity test (MIT) at 320 psi. Due to holes in the storage tank the well was not used and shut in during this time period. The tank was replaced and the operator was requested conduct an MIT to verify the integrity of the well. The well was initially pressure tested and held at 325 psi. The permit conditions required the testing of the well at 850 psi. The well was pressured up and tested at 890 psi with zero pressure loss. The well passes and is in compliance. The flow line was pressure tested and held pressure at 440 psi. The flow line was retested to be incompliant with the maximum permitted pressure. The line was pressured up to about 1000 psi several times and rapidly lost pressure. The line was disconnected from the well head to eliminate variables such as a leaky valve; the pressure did not hold. The operator was advised injection of fluid into the well could not commence until a plan was established to remediate the issues with the flow line.

The operator excavated the line to locate and identify leaks. A pin hole leak was noted about halfway from the tank to the well. This section of pipe was removed and replaced with a new section. A line pressure test was conducted and failed. Various fittings were replaced and tightened to eliminate potential leaks. The line was retested and failed.
