0158880250 2016-05-13 Final Restoration No Violations My inspection today is a status check on a final restoration. During my inspection I witnessed no evidence of an oil and gas well and the site has been restored. Location has met 1509.072 B requirements.
0219078128 2014-12-31 Plug / Plug Back No Violations The ridgetop has seed a hay and shows signs of vegetation, however the access road leading up to the location is full of ruts and barren areas. I will contact Casey with Artex to discuss the issues.
0328847590 2014-05-31 Drill / Deepen / Reopen No Violations Witnessed 687 feet of new 9 5/8 surface casing set in 730 feet of 12 1/4 borehole with 235 sacks Class A cement. 75 sacks lead cement at 2% gel and 160 sacks class A neat cement. Circulation established and good cement returns were seen at the surface and stayed up. 12-15 Barrels into the mix of the lead cement the pump truck encountered an obstruction in its valves, lines were disconnected and flushed before pumping resumed. While the pump was disconnected circulation was lost, and the fluid level dropped but returned shortly after the pumping resumed.
0765913775 2014-07-23 Plug / Plug Back No Violations I was on location to witness Berry Well Services test the 7 casing at 500 psi for 15 minutes. Timco Inc then ran a cement bond log and tagged top of 7 plug at 3029. The top of cement on the 7 annulus was at 2550 and they shot the pipe at 2300. The 7 did not break loose and freepoint was calculated around 1400. Timco then shot the 7 again at 1396 and operator was unable to free the pipe. Timco shot the well off twice at 714. The bottom of the 9 5/8 is at 687 and the 12 1/4 is at 730. Berry Well Services then pulled the 7 casing from the hole and ran 1.5 tubing back down to 1400. They will return tomorrow to set their plugs.
1401638134 2014-06-01 Drill / Deepen / Reopen No Violations Witnessed pressure tests for the surface casing and the blowout preventer(BOP). The surface casing test was combined with the blind rams on the BOP. The test pressure at the beginning of the test was 500 psi. Test was ran for 30 min. At the end of the test the pressure was 460 psi. The test passed. After that, drill pipe was tripped in and the annular section of the BOP was tested. The company pressured up to 500 psi. and held it for 15 min. The pressure did not drop. This test also passed.
1401711319 2014-05-30 Casing String Witnessed No Violations I was onsite to witness the conductor behind cemented to surface. Kilbarger rig #17 is drilling for Artex Oil Company. There was 85ft of 13-3/8 ran in the well and cemented. The cement dropped 30ft after we stopped pumping. We let the cement set up for 30 minutes then pumped the cement down the backside to the surface. The cement stayed at the surface. The conductor is cemented to surface.
1402960541 2014-06-16 Plug / Plug Back No Violations Well is a dry hole and was plugged on active drilling permit. Witnessed bottom hole plug set from 6301-6101. Witnessed Pt. Pleasant plug set from 5574-5374. Witnessed Clinton plug set from 4268-4006. Witnessed bottom of the 7 intermediate casing plug set from 3152-2952.
1406218390 2014-07-24 Plug / Plug Back No Violations I was on location to witness Formation Cement set a Berea plug (1266-1466). The tubing was set at 1466. Berry Well Services then pulled 21 joints of 1.5 tubing (34.10) up to 750. Formation then set the surface casing plug (587-750). Fluid level was around 250 and operator pulled remaining tubing from hole. A brush and stone plug was set at around 250 and fluid stayed at around 200. Formation pumped the surface plug (0-200) but the plug did not come to surface and fluid level was still at 200. Operator will let the well set overnight, tag the top in the morning and set plug to surface.
1406291310 2014-07-25 Plug / Plug Back No Violations I was on location to witness Formation Cement top of the hole with 100 sacks of Class A cement. The fluid level at arrival was at 200. The cement stayed at the surface.
1962381485 2014-08-15 Drill / Deepen / Reopen No Violations I was on location with Artex personnel Frank Wriggle and Casey Fritz to review final restoration requirements. Grass is growing around the area but there are still erosion issues with access road. I will continue to monitor the location.
