0391502034 2013-11-25 Plug / Plug Back No Violations Artex drilled to TD however once they hit the Trenton the drill bit started to build ~1 degree/100. They ran their logging and directional tools after they reached TD and found out they were off target when plotted back onto their seismic map. This was most likely due to the structural nature of the subsurface geology which caused the drill bit to drift. Artex set two plugs (6518-6118), (5784-5584), and a kick-off plug (5279-4779) using Universal Well services and will return on Monday after Thanksgiving to resume drilling operations.
0468116476 2013-12-02 Administrative Inspection No Violations Jason Ritchie of Artex called and stated they had moved the Kilbarger Patterson Rig 17 to the next well due to a slip on the far side of the location. They believe this was caused by the recent wet weather so they equipped the well with a reducer swedge and will return to drill to TD when the weather has dried out or frozen the location. They are going to drain and haul away the fluid in the pit within the next few weeks.

The well pad has been restored to a useable surface, however there are still signs of slope instability. They are beginning to bring rig equipment on location. The rig hand told me they will not be using a pit but will be drilling on fluid and using a storage tank.
1383217658 2014-07-02 Drill / Deepen / Reopen No Violations I was on location with state engineer Jason Simmerman. Contractor was on site using backhoe and dozer in order to stabilze the area to avoid further slipping of the well location and slopes.
1384426892 2013-11-14 Drill / Deepen / Reopen No Violations I was on location to witness the cementing of the surface casing by Formation Cement CO. Artex is using the Kilbarger Patterson #17 rig. The Kilbarger rig drilled a 12 1/4 hole to a TD of 741. The surface casing shoe depth was 688. Three centralizers were placed on the surface casing and the baffle insert depth 645. The plug was down at 815am with good cement returns however they used a composite baffle which broke just after landing the plug. Cement dropped ~10 feet. I witnessed Heath, of Artex, set up the pressure gauge and meter box later that afternoon and tested the annular up to 650 psi. Both dropped to ~530psi and held steady for 20 minutes. He will also perform a BOP test later tonight or early tomorrow morning.
1386098226 2013-12-03 Drill / Deepen / Reopen No Violations This was a location inspection in regards to a slope slip on a section of the well pad. The back right hand side of the location had slump and an area on the shallow side of the pit had slipped down hill due to the recent wet weather. The call was made to remove the rig and all equipment from area. Upon inspection, I found the well shut in with a swedge and the operator had graded the location. The pit still had fluid in the deeper end. I called Jason Ritchie of Artex and he stated they would be hauling away the pit water by the end of the week. They are not going to resume drilling until the ground dries out, which will be sometime next year. Within this time period they will begin working on rebuilding the damaged area. I will continue to monitor the location.

1387211367 2013-12-16 Drill / Deepen / Reopen No Violations This was an inspection of the well location. There is a fluid storage tank at the top of access road equipped with a hose running down into the pit. The pit still has fluid, however everything is completely frozen at this time. The side of the pit has fallen away some more, but Artex has recently used heavy equipment to access the bottom area where work is needed to properly restore the location. I will continue to monitor the site and keep in contact with Artex until the area is secure.
1387379583 2013-12-18 Drill / Deepen / Reopen No Violations Kyle Baldwin and I visited the location to inspect the slope stability.
1404141765 2014-06-30 Drill / Deepen / Reopen No Violations Kastelic Dozer Service was on location moving dirt to various areas. The operator has recently excavated a large water diversion trench at the base of the outcrop in order to move the water away from the well head area. More of the slope has fallen away from the front slope and there are numerous cracks running right up to the wellhead. The operator on site was trying to move dirt to this area and then compact it to rebuild the location. They plan on working this are until it is good enough to move the rig back in to finish drilling the well.
1408467541 2014-08-19 Drill / Deepen / Reopen No Violations Bishop Well Services was on location and there is a lined pit adjacent to the wellhead. Jason Ritchie called last week and said they were completing the well. Well was currently shut in and noone was on location.
1420037922 2014-12-31 Drill / Deepen / Reopen No Violations I was on location for a preliminary restoration inspection. Artex has filled the pit and graded the entire location. Operator had a crew out spreading pasture mix and hay all over the area. There is nothing growing at this time due to the weather, so I will retun in the spring to check on growth of vegetation. I was told that Artex plans on putting a pumpjack on the well. It is not currently in production and hooked up to a steel tank.

1439230501 2015-08-10 Plug / Plug Back No Violations Set 150 ft rip plug at the 4.5 inch rip. Then pulled up to the bottom of the 7 inch and set a 200 ft plug.
1439392508 2015-08-12 Plug / Plug Back No Violations I witnessed Bishop Well service rip the 7 inch at 2146 ft. Then Formations Cementing set a 100 ft. rip plug. Bishop Well Service pulled up 24 joints and Formations Cementing set the 200ft. Berea plug. Letting plugs sit over night resuming tomorrow to finish plugging.
1439486170 2015-08-13 Plug / Plug Back No Violations I witnessed Bishop Well service tag the Berea plug at 1222 ft. Formation Cementing then arrived onsite to set the 200 ft. bottom of surface plug. Bishop Well Service then pulled 7 joints and Formations Cementing set the final surface plug from 215 ft. to surface. Bishop Well Service then pulled the remaining 7 joints and the plugged remained at surface.

1693208159 2014-07-24 Drill / Deepen / Reopen No Violations Artex moved the Killbarger Rig 17 back onto location to finish drilling to TD. They tested the pipe rams on the BOP at 1000 psi for 15 minutes with no issues. They then tested the annular at 900 psi for 15 minutes with no issues.
