1045156286 2012-10-10 Production Wells No Violations I received call from Operator regarding this oil spill. I have now inspected clean-up and found it is complete. The pumper was on location today picking up the remaining pads from the creek and dam will be removed from creek today or tomorrow. I e-mailed OEPA to let them know status. Pumper stated that well was still down, due to repair of time clock on pumping unit so we did not visit well today - Jake Glascock also present.
1296904121 2012-10-10 Production Wells No Violations I accompanied Cindy VanDyke on the inspection of well. We did not inspect well however there was 1/100 barrel tank on site and dike was in place. We inspected surrounding area and creek for remnants of oil spill and found no contamination. Pumper was on site and cleaned up remaining diapers and stated that pump jack was down due to faulty time clock.
1346065985 2012-08-15 Production Wells No Violations I received call from Dan Goins that oil spill had occurred. I called Dick Poling back and claimed he had it all cleaned up - says it happened over the weekend - timer on well stuck and when well did not shut down, ran the tank over. I asked him why he did not call me when it happened and he said he was too busy cleaning it up - isnt that what he wasnt supposed to do instead of talking on the phone? He said oil had just barely got into the creek. I said, NO, he was supposed to call the inspector, 1st thing. He said he had lost 10 bbl which he had since recovered - inspection this day found oily ground was gone, dozer had built a insufficient dike around the tank, and oil had travelled downhill from the tank into the creek that runs along the road. I asked him if he had called OEPA and he said NO, so told him to call them this day. I found oil had travelled down the creek about 600 and found pads still in the creek with a dam across the creek at the end of the spill. Most oil was gone from the creek but pads still in place to pick up any remainder. Spill was reported to Poling on Monday morning, they spent Monday and Tuesday cleaning it up before calling Tuesday evening. Identification complete on the tank, violation to follow with next inspection, for insufficient dike.
1346158163 2012-08-27 Production Wells No Violations My inspection due to oil spill reported last week. On 2nd viewing of dike around tank, believe it barely sufficient to meet SPCC standards. Fresh, clean pads have been put down in creek, no pooling of oil observed in the creek and complete identification posted on tank. Also saw some fish swimming in the creek.
1378741026 2013-09-09 Production Wells No Violations This was an inspection at the request of the Columbus office Division of Oil and Gas Resources Management. Upon inspection, I found well to be currently in production. The well is in the process of being transferred to exempt domestic ownership and there is a new home being constructed around 150-200 from the wellhead. There are no contamination issues at the tank and there are no issues to report at this time.
