I called Dick Poling and told him the tank was mislabeled and he stated they would come out and correct the permit numbers. It is currently labeled as 24550 on one side of tank and 7945 on other side. Correct well ID is 24591.

Operator needs to seed and mulch well site and lease road. The spud date was 9-13-11. Coordinates for well and tanks were reported on earlier inspection. I talked to Dick and he said the dry weather last year killed all of the seed they put down and after a good rain they will put more seed and straw on ground. Coordinates are on earlier inspection.

The site needs remedial restoration work and after talking to Dick Poling, he stated his son would be out there in the next few days to take care of situation. The well was found producing with no other apparent issues at location.

1373907469 2013-07-15 Production Wells Violations Noted Dick had asked me to head over to see if grass had starting growing back in order to sign off on restoration. He had mentioned that they had a line break and a small oil leak. I arrived on location to find that they had repaired and reburied the line however there was a small area of oil contamination covered with adsorbent diapers. I called Dick and he said he thought his field person had taken care of the cleanup and said he would get someone out there this week to remove the contamination. He is having a tough time getting location to grow vegetation due to the red clay. The coordinates for this well are found on a previous inspection. Site does not yet meet restoration requirements.
