0159403574 2017-07-11 Production Wells No Violations I was on location today to do a follow-up inspection on this well. At this time the identification has been fixed and is on the production tank but the heater tube is still leaking on the tank.

1444051330 2015-10-05 Production Wells Violations Noted On the date of my inspection I found the identification to be faded enough that I could not discern the API # of the well or the lease name. I also noticed that the heater tube has leaked and allowed oil and or brine to leak from the tank and onto the ground. Tapping the side of the tank indicated low fluid levels in the tank. The visiable affected area was about 2 by 3 below the heater tube. The valves were open on the production lines at the well head. The well was equiped with an electric motor which was connected to an electric box at the well site. The electric motor was on a small Jensen pumping unit. Repair or replace leaking tank. Clean up and remove all oily material from the dike area and from around the well head and dispose of in a proper manner. Correct the illegible identification sign found at the well on the tank. No production records have been reported to the state since 2011, submit productions records to the Columbus office for 2012, 2013, 2014
1451397916 2015-12-29 Production Wells No Violations Follow up on the NOV on this well found that nothing had been done to the leaking tank. It is still leaking and the mess still remains on the ground. The only violation that was corrected was the ID. The tank still needs repaired or replaced the contamination on the ground needs remediated. Rain water entering the tank seems to be flushing more oily substances out of the tank through the heater tube. This inspection referenced inspection # 1444051330.

2141600823 2018-10-02 Production Wells No Violations I was back on location today to do a follow-up inspection on this well. All violations have been corrected. The well is no longer in violation.
