0029303393 2011-10-18 Production Wells No Violations Inspection this day due to complaint by landowner, Debbie Hivnor, about some line leaks and chronic, long term problems with this operator. Agreed to meet with her to address/dismiss the complaints. Showed me 2 recent line repairs that she said were still leaking - I found them wet on the underside but no drips - asked pumper to check once a week to make sure they dont get any worse - not significant at this time. Also wanted me to look at 2 past sites - found cows favoring these spots and slightly salty and asked operator to put 4 sacks on one, one on the other to sweeten soil more. If briney continues, may have another leak. Wanted to blame dead trees on these past line leaks - I could not confirn/disprove that. Also wanted me to look at small plastic water tank - said it was dripping or leaking but I dug under valve and found no drip - ground wet and slightly salty due to wet weather and typical tank dike conditions. Told her nothing unusual or that needed repair. Ongoing battle between landowner and operator for many years now - general conclusion was that complaints unjustified. RESOLVED

0172638309 2010-09-07 Production Wells Violations Noted An inspection revealed a 2 flow line parted in a wooded area, it then flowed into a small dry run approx. 300ft. It appears approx. 1 bbl of oil has flowed from line into dry run. Spoke with Joe Altier and explained he should have notified Ohio EPA of spill. Joe Altier said flow line has been repaired and he felt approx 5 gallons of oil had been loosed. I notified Dennis Deavers with Ohio EPA and will meet him at site 9/8/10. Explained to Joe Altier he needed to clean up all oily material and dispose of in a proper manner.

0337564457 2018-05-25 Production Wells Violations Noted I was contacted by the land owner Debbie Hivnor letting me know that there is a leak in the middle of one of her cow fields. I arrived on location and contacted Joe Altier to let him know about the situation. Joe showed up with his pumper Jason to clean it up. Today they will be putting down pads to soak up any free standing oil. They have an excavating crew lined up to come in and remove any contaminated soil on Tuesday. There is less then 5 barrels of contamination that released.

The grass was growing but the landowner has put her cattle back in the field. They have walked through the grass and tore it up. I will stop back at a later date to check on it again just to monitor it.
0673459439 2015-07-14 Production Wells Violations Noted I was out at this tank pad to check on an oil leak coming from the production tank. The second tank in the dike is leaking oil from the bottom of it. The tank will need to be replaced before any further production can continue. The overflow line was also open and water was flowing through it. I tested the water at 578ppm. The contaminated soil inside of the dike and outside of the dike will need removed from the location and fresh soil will need brought in to replace it. Joe Altier has had all of the contamination removed from the location. He fixed the flow line where it busted and then brought in new top soil to replace what he took out. The area has been reclaimed and put back to its original state. There are no violations at this time.

1055182480 2003-06-06 Annular Disposal Violations Noted Inspection of the pipeline connecting this well with P# 4197 in response to a complaint by the landowner was conducted 6-6-2003 with Tom Tugend. The pipeline has ruptured again in area previously repaired just a week ago. Oil has saturated the surface in area approx. 20 by 50. Numerous spots are spouting gas and oil/brine. JV Altier was on site and given specific instructions: Repair and perform a pressure test an the entire line; or, replace the entire pipeline from the well head to the tanks of P# 4197. Mr. Altier was given instruction to immediately begin clean up. He advised he was scheduling Steve Altier to begin next Wednesday. Mr. Altier was informed this was unsatisfactory, the clean up was to begin today. We provided him with a list of contractors and insisted he make arrangements immediately. Mr. Altier finally contacted Dave Blosser who would able to be on site this afternoon. Mr. Altier was advised to remove the oil saturated soil, haul it off the site and regrade, seed and mulch.

1327974707 2012-01-27 Production Wells No Violations Debbie Hivnor called at 6:30 PM to report another oil leak at this site and stated that Joey and the pumper are on their way to the location. Later, Joey called to say the situation was under control, that the well had been shut down, less than 1 barrel of oil had been released and nothing had reached the stream. He said they will start clean-up in the morning. 1/27/12 - I inspected the spill location and found oil seeping out of the ground and has run downhill approximately 20. It has been so cold out the oil did not travel very far. The cleanup crew was out today collecting any free oil with sorbent pads. Joey said they will not be able to repair the flow line leak for several days and will keep the well shut in and no pressure on the flow line. On the last release, Joey had replaced a length of flow line from well to this spot in pasture. This leak was in the section that was not replaced. Well owner is considering replacing the steel flow line with plastic.
1438097422 2015-07-28 Production Wells No Violations I was contacted by Debbie Hivnor letting me know they found a small leak on the AD flow line. I met with her today then went out and found the flow line. When I got to it I found it had already been fixed by the company. Once I did that I contacted Joe Altier and had him meet me out at the tank pad so we could go over the compliance notice that was issued on it last week. We will be monitoring the production tank for the next week to see if it is leaking or not. If it is he stated he would replace it.

1546269748 2011-09-11 Administrative Inspection No Violations Joe called to say a tree had blown over during a recent storm and fell on water line leading from separator and damaged it. Allen (landowner) had been out to check on things after the storm and found it. Joe shut well down and all water going into tank now. Joe says he will repair problem. 9/12 - Joe called to say line has been repaired and well is back in production.

1709814057 2015-08-14 Production Wells No Violations I returned to this well today to do a follow-up on a previous violation for the tank leaking. Since my lays visit to the well the land owner contacted me and told me they had removed the bad tanks but did nothing with the soil in the dike. I let her know I would be down to take samples of the ground to see if it was contaminated. Upon arriving at the production tanks I used my shovel and removed some soil from the base of the tank. Once I did that it opened up a seam where oil poured into from the surrounding area. I contacted Joe Altier and had him meet me down at the well. They started cleaning up the ground right away. They dug an 8 area approximately 1 deep and bailed out the heavy oil. Once they got that cleaned up I had them put down some pads for any oil that seeped out of the ground. Joe is trying to line up a water truck for Monday morning. We will be flushing the ground under the tank to make sure there is no more contaminated soil. All of the old pads and contaminated gravel will be loaded today in the back of their Kubota. The estimated oil loss was approximately 1-2bbl. The new tank was also leaking from the bottom of the bull plug. That issue has been resolved.

1796668141 2012-02-09 Administrative Inspection No Violations My inspection due to conversation with Joey Altier - I was questioning whether the spill on this well has been cleaned up. Joey says he has not replaced the line that leaked yet because it has been too muddy - that he would tear up a lot more than he needs to and will wait until dries out a bit. He still has the well shut in and the pads are still down on the area collecting oil. He says he has removed the dam that they were complaining about and that he has been locked out of the gate going into the lease by the Hivnors.

Joe Altier has used absorbent materials to soak up any free oil on the location and has put down some straw in case it would rain. They will be bring in equipment to remove any contaminated soil and will replace it with top soil.
2104228712 2011-08-23 Production Wells No Violations Inspection this day is follow-up on small oil spill - found Joe Altier on location, supervising laying of new plastic line from well to place in hollow where small hole (leak) was found. Should have new line all laid by 8/25. Small bit of oil surrounding dug hole where leak found, sheen in creek above and 50 below dam (dam not holding completely) - heavy rains today(8/25) may hinder completion. Joe has assured me all oil will be gone, just hadnt got to that point this day. With this replacing of line, all the line should be new between well and tanks. Debbie Hivnor had called, wanting me to tell them to move the tanks over by the well and told her I could not do that.
