0311381781 2012-03-02 Annular Disposal No Violations On the day of my inspection I found a pumping unit at the well site, the well could be capable of production, the permit number is posted on the power box at the well site, the annular is not exposed, there are (3) one hundred barrel production tanks/knock off/ (1) brine tank/ separator, the tanks are inside a containment dike the separator is not.

1332253231 2012-03-07 Annular Disposal No Violations On the day of my inspection I found four inch well casing with tubing and gate valve standing out of the ground, the annular on the eight inch was not exposed, there is no permit number posted at the well site, there was a gas leak at the well head, there are (2) one hundred barrel production tanks/ seperator/charts, the permit number is posted on the tank.
1372795798 2013-07-02 Plug / Plug Back No Violations Arrived on location to witness plugging of Knox Energys Diocese of Steubenville #3B. Plug job was performed by Berry Well Services and cement job was completed by Cemenco Services Inc. Berry well services had to bull head plug the bottom hole plug due to paraffin issue in the tubing. Cemenco set a 400 bottom hole plug using 30 sacks of Class A cement from 1200-1600. Telegraph was found at 394 due to slim hole conditions. They ripped the 4.5 casing at 394 and pulled the remaining 9 joints. Cemenco then set a 350 plug from bottom of surface casing to the surface using 60 sacks of Class A 2% cement. The cement stayed at the surface.
