0438406402 2016-11-07 Final Restoration No Violations This well was plugged and abandoned earlier this year. The inspection today revealed that this well has had all equipment removed, vegetation has been established, the pit has been closed and buried, and erosion has been controlled. This well has passed final restoration in accordance with ORC 1509.072(B).
1295555254 2016-07-28 Plug / Plug Back No Violations I was on location for well plugging. 1.5 inch diameter tubing was set at 698 feet and Formation Cementing pumped a 400 foot bottom plug inside of 4.5 inch diameter casing using 30 sacks class A cement and 1.5 barrels of freshwater displacement. Tubing was pulled out of hole and Appalachian Well Surveys was brought in to determine free point on the 3.5 inch diameter casing and shoot it off. A cement bond log revealed that the well had a no issues on the back side of the casing. Casing was shot off at 252 feet and we tried to pull the casing out of the hole. The weight indicator showed that a lot of weight was still on the pipe. We believed that when we shot we did not part casing and we almost had one joint out of the hole but the rig could no longer pull the casing. We left all the casing in the hole and put a solid plug from 252 foot to surface. This well has been plugged to surface and all violations have been resolved.
