0000PL9464 Preliminary Restoration No Violations Preliminary Restoration done information from Data Point
1485845663 2005-02-03 Production Wells No Violations No violations noted on the day of my inspection.

Well equipped with 2-210bbl production tanks shared with another well pn#3795.

1495458134 2017-05-22 Plug / Plug Back No Violations I was on location today to do a plug back on this well. Artex Oil Company used Bishop Well Service to do the service work on this well and Formation Cementing to pump the cement in the well. Bishop ran the tubing in the well down to 6,948 feet to set a 30 sack 400 foot plug on top of the bridge and cement plug that was already there. They are closing off the Beekmantown Formation. Once the cement plug was set the rig crew pulled the tubing out of the well.
1660629282 2017-01-19 Production Wells No Violations The inspection this day revealed that this well is equipped with a pumping unit, a 4 1/2 inch diameter wellhead, 1 1/2 inch diameter production tubing, and 2 inch diameter steel production lines. Near the wellhead there is a meter box with a current chart in it. The valves on the wellhead are open at the time of inspection. The tank for this well is at the entrance of the well. The tank battery consist of four (4) tanks two 210 barrel steel tanks, one 100 barrel steel tank, and a 50 barrel plastic tank. Two tanks, a 210 barrel and the 100 barrel tank, are labeled for the Clarence Hess 4-31 leaving me to believe they are used strictly for that well. There is a single 210 barrel labeled for this well with proper identification but is in the shared tank battery. There are no issues to report and the well is in production at the time of inspection.
2098653737 2015-10-07 Production Wells No Violations Upon my inspection today I found the well to have no violations at this time and is in proper working order. The well is equipped with a pump jack and is capable of production.
PL9464 PL No Violations
