0122277331 2018-04-12 Production Wells Violations Noted I was on location today to follow-up on the clean-up of the tank pad. One of the production tanks has leaked into the dike and has caused some contamination. Nothing has escaped the dike at this time. I have contacted Bill Grizer and they are getting stuff lined up to get it cleaned up. They are planning on bringing a dozer down and a vac truck to clean up the dike. I asked him to keep me posted on it. I will check back as needed until the contamination has been removed.

I talked with bill Grizer today and he said they would be getting the identification posted. I reminded him it had to be at the well and the tank pad for multiple wells.
0233223943 2018-07-09 Production Wells No Violations I was back on location today to do a follow-up inspection on this well. Bill Grizer has soaked up all of the fluids out of the dike with a vac truck. He has also put down different contamination absorbents to soak up any free contamination. I will return back in a few days to check it again.
0644505962 2015-07-20 Production Wells No Violations I was at this well to do a follow-up inspection on it. The well is still in violation at this time. The company has been working on getting the violation fixed. They have built a new dike and are setting new tanks. With all the rain we have had they got delayed on setting them because they can not get trucks in . They plan on having it done as soon as possible. We have the company as Columbus Oilfield Expl. Co. in out system and the company that is operating and selling the production is C&G Oil and Gas. The owner is Bill Grizer and his contact number is 740-525-3586.
0657775573 2011-10-11 Production Wells No Violations Inspection this day found Ray Branham down at well, working to clean location for rig to move in. Says he owns the well, altho C & G Oil and talked to landowner, Dustin Kinney and he says Rays lease is not valid. Ray believe C & G Oil will work on well, share half the profits. Well has jack, reportedly rods have pulled apart - was a good well but hasnt produced in long time. No IDs, separate sitting along side the lease road and not connected.
0701443076 2012-05-17 Annular Disposal Violations Noted AD inspection found AD not hooked up or in use with the following equipment present -4.5 production casing -1.5 tubing and the production valves open and the well being produced by the means of a pumping unit -inspector will note well has contaminated area around well head that needs cleaned up , 3 210 barrel tanks and 2 separators shared by other wells on lease with identification present ,Inspector left word with company to clean up paraffin and oil -get Ids up on wells and plugs in outlets on production tanks ,Lead inspector Cindy Vandyke stated that Carper well Svc out of Marietta recently took over wells and is in the process of getting them registered .Inspector will follow up in 60 days .

The wellhead has the valves open on it and the well is capable of production at the time of inspection. The tank battery has two 210 barrel tanks and two separators in it. The battery appears to have several wells going to it. There are no issues to report at this time.

1029056642 2015-03-31 Production Wells Violations Noted C & G Oil and Gas are the owner/operators of this well. The well is currently in production and is equipped with a pumping unit. There are three 100 bbl tanks less than 20 from the pumpjack and there is no dike. I talked to the field supervisor, Bill Grizer, and explained that the tanks need to be a minimum of 50 from the wellhead and inside an SPCC dike. He said once the ground dries out a little more they will be able to get down to the area and correct the issues.

At this time the well is still not cleaned up all the way. The straw and pads that the company put down is still in the dike. The contamination is still contained in the dike at this time. I will talk with Bill Grizer again to see what the plan is on this.

Someone likes to poke holes in line, causing well to be more oily around it than should be. No leaks this day, valves open and well equipped with pumping unit. Well has been cleaned up around from last time it was looked at. AD inspection in the past found one of the leaks. AD not being used.
1350658010 2012-10-10 Production Wells No Violations I accompanied Cindy VanDyke on the inspection of well. There were 2 laydown separators and 3/310 barrel tanks. ID was posted on tank, and the well is capable of producing with no apparent issues at location.
1388885970 2015-04-07 Production Wells No Violations Compliance Follow-up 1029056642: I arrived on location and found that Bill Grizer of C & G had removed most of the contamination from the first area. He had limed the ground and threw down some seed. I called and told him that there were still issues with area one and that there he also needed to remove the contamination and restore the vegetation from area two further down the pipeline access. He said once the rain clears up he would get out there and keep working on issues.

There is still contamination in the dike so I called Bill Grizer again about the contamination and he told me he would have it done by September 19th.
1498973887 2018-09-21 Production Wells No Violations I was back on location today to check on the clean-up of the contamination at this well. I was told by Bill Grizer that he would be back by 9/19/2018 to get the area cleaned up. I walked down to the tank pad today and they were not on site. The contamination is the exact same from last week. I Contacted Mason the production supervisor and talked with him about the contamination. He told me he was not even aware of the problem until he talked with Eric Conrad the day before. He is planning on coming down today to see what he needs to do.

1527706071 2018-05-30 Production Wells No Violations I was back on location today to follow-up on the contamination located in the storage dike for this well. at this time the dike is still full of contamination and needs to be cleaned up. I tried to reach Bill Grizer today but I was unable to during my inspection. I left bill a message for him to call me. When I last talked to him he told me he would be down there that week to finish cleaning it up but it does not look like they have returned to the location. Bill ended up calling me back later in the day. He let me know that he would be back at the well in the next day or so and get the violation fixed.
1529567330 2018-08-22 Production Wells No Violations I was back on location today to do a follow-up on the contamination that is inside the dike for this well. At this time there is contamination still inside of the dike around the storage tanks. I have taken updated photos to send to my supervisor to discuss further actions with the company.
1542117652 2018-11-13 Production Wells No Violations I was back on location today to do a follow-up inspection on this well. All of the contamination has been cleaned up and removed from the containment dike. The production tanks that were not in use have been removed from location and a new motor has been put on the pumping unit. The only thing that still needs done is the identification for the well. Once this is done I can pass the compliance notice.

The company was not on location while I was there but they have removed and put all of the contamination in a couple of 55 gallon drums. I contacted the company and let them know everything was good. As soon as they get the drums off of location I will pass the compliance notice.
1739907871 2018-10-03 Production Wells No Violations I was on location today to witness the cleanup of the contamination on this well. The company had a dozer on location to redo the lease road and to clean out the containment dike. The contamination was put into 55 gallon drums so it can be hauled off of location. The crew did not get everything done today so they will be back in the next day or two.
1851882022 2019-04-02 Production Wells No Violations I was back on location today to follow-up on this well with the company to see if they had any progress with Columbus on getting the wells under their bond. They have been operating these wells and reporting production for years now. They stated to me that all of the paperwork has been submitted to Columbus and they are just waiting. I will follow-up with my supervisor on this.
