1239156342 2015-03-13 Administrative Inspection No Violations Joe Schnieder from Artex called and stated that they gave the pipe to the adjacent landowner but he has not been able to get it removed due to the weather. He will call me once the pipe has been taken off location so we can pass final restoration. The land is owned by AEP.
1342669612 2014-03-10 Plug / Plug Back No Violations I was on location to witness Formation Cement Company set a rip plug (2615-2815), surface casing plug (1654-1854), and a surface plug (0-200). There was circulation on all plugs and some oil. They used a total of 160 sacks of Class A cement.
1394133982 2014-03-06 Plug / Plug Back No Violations Bishop well service is on site and Artex is plugging well. Timco is perforating thru 2.375upset tubing. They will run thru and drop out the bottom of the tubing at 3950. They will shoot 15 holes at .460 and perforate the 4.5casing. Then the rig will run in with tubing to top perforation and set the bottom plug at 5221-4871. They will then pull up to the perfs at the lime and set the lime plug at 3950-3650. Circulation was established on both plugs. 50/50 poz was ran on both plugs. The highest H2S reading was at 65 ppm when pumping on the well.
1394546051 2014-03-06 Administrative Inspection No Violations Steve Miner with Artex called on tagging cement on the Big Lime plug at 3060. They ripped 4.5 casing at 3024. They pulled all 4.5casing out and will run 1.5tubing in and set 200 rip plug.
1426091939 2015-03-11 Plug / Plug Back No Violations This well was plugged a year ago and upon inspection I found the location was growing vegetation. The production/flow lines had not been removed along the access road up to the well. Operator needs to remove the production lines in order to pass final restoration.
1503891572 2014-03-10 Plug / Plug Back No Violations Bishop well service ran 1.5tubing in to depth of 2815. Artex set rip plug at 2815-2615. Ran 50 sacks of Regular Class A cement at 2%gel. Bottom of surface plug was at 1856-1656and ran 50 sacks of Class A cement at 2%gel. 200-0 was set at surface and ran Regular Class A cement at 2%gel. All plugs circulated.
1811948033 2016-05-16 Final Restoration No Violations My inspection today is a status check on a final restoration. During my inspection I witnessed no evidence of an oil and gas well and the site has been restored. Location has met 1509.072 B requirements.
