1265672195 2017-12-15 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. I am conducting a follow-up inspection on Chiefs Order 2017-101. I found the well equipped with casing, tubing, and a cut flowline attached to the tubing. The well has a pumping unit with an electric motor and proper identification. Pictures were taken during inspection.
1426378194 2015-03-14 Production Wells No Violations Inspection on this day revealed that fresh water had filled up area between wooden conductor pipe and 7 surface casing and let oil escape onto the ground and it entered a small wet weather stream that feeds Rich Fork Creek and estimated release was 1-2 bbl of oil and the Monroe county EMA contacted local inspector Bill Stolarik and I received call from him at 12:00 pm asking if I would look into the matter and I arrived on site at 1:15 pm and located problem well and then contacted well owner requesting his presence at site to begin clean up efforts on spill, instructed owner to remove any contaminated soils and properly dispose of it at a EPA approved landfill.

Note: Equipment for clean up arrived on site and soils were placed onto plastic liner and covered for the night and then they started to work there way down stream to clean up small pockets of oil in creek.

1426512539 2015-03-15 Production Wells No Violations Inspection on this day indicated that contaminated soils were still on plastic containment and crew was trying to haul material up the hill and place into roll off container and I walked entire creek bed to check area for any free standing oil and on this day very little was found and I told cleanup crew to change out absorbent pads twice daily until given father notice.
1693303309 2017-12-15 Production Wells No Violations Order 2017-101 follow up inspection found the well equipped with casing tubing and pumping rods, the well is set up to be produced by the means of a pumping unit. There is a casing and tubing well head present with production valves in the closed position and no flow lines attached. There is no production tank present associated with the well and no commercial gas meter present on the lease.
1746527227 2015-03-15 Production Wells No Violations Administrative report to document the situation with a oil spill at the well head that entered the waters of the State. At 11:32 Am on 03/14/15 I was notified by Phil Keevert with the Monroe Co EMA that they had an oil spill of approximate 50 gallons that had entered the waters of the state and the EPA was also notified and enroot, I notified my Supervisor Brent Bear and contacted Inspector Dave Ball to respond. I also made contact with the Steve Sapio with the USDA who is the area inspector for the US Forest Service and reported the spill to Mr. Sapio on this day. I remained in contact with Inspector Ball and Supervisor Bear through the course of events of the day documenting the situation. 03/16/2015 The initial well identification reported by the County EMA Director was found to be incorrect- I have correctly identified the well as 3411165719.
1837868187 2015-03-15 Production Wells Violations Noted Status check inspection of the well head found the following equipment present at the well head 6.625 casing 2 tubing and pumping rods with no production lines attached to the well head. A small fuel type tank is located near the well head as a production tank but not in use. ODNR-RBDMS records show no reported production in the database. Instructions are as follows-Plug the said well within 180 days -ODNR Monroe County Inspector Bill Stolarik- 740-507-6592.
1981067505 2017-11-13 Production Wells No Violations Upon inspection of the well I used my State issued Garmin GPS device and took a GPS reading of the well of EPE 11-39.73039 X 81.17933 and confirmed the well identification as the Sloan # 3 owned by Cisler Oil & Gas ODNR owner # 5719. Also noted is Historically the well has a unresolved violation issued against it on 03/15/2015 with the violation identified as 1837868187. The well was observed to be equipped with at surface 6.625 diameter casing with a casing valve present and no flow line attached. The well has 2 diameter tubing inserted inside the production casing with a tubing production head present and no valve or production flow line attached to the tubing head, only a broken off piece of clear plastic sight tube. There is pumping rods inserted into the 2 diameter tubing and the well is set up to be produced by the means of a pumping unit with a electric motor present on the unit. Also noted is the absence of a production fluid tank or gas sales meter on the lease that can be identified as associated with the said well. Photographs were taken on this day and recent attempts to contact the registered producer identified as Scott Cisler has not been returned.
2097318503 2012-07-16 Production Wells No Violations Inspection of the well head found the following equipment, 6.188 surface casing, 2 tubing & rods, with the well equipped with a pumping unit and gasoline motor, no production lines hooked up and a 800 gallon vertical tank sitting next to the well head. Well identification present.
