0698467737 2018-03-26 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The well is quipped with 6.625 inch diameter production casing with a inner string set inside the production casing as a second production string. There is a 2.375 inch diameter tubing inserted into the inner casing string. The annular space between the inner and outer casing is open, and it appears to have minor gas bubbling. It is produced by means of a pumping unit with an electric motor, tubing, and rods. There is (1) 70 barrel steel tank and (1) 100 barrel steel tank. The production records are current, but 0s have been reported since 2015. Awaiting production numbers for 2017.
1480113109 2017-01-17 Production Wells No Violations Status check inspection found the following equipment present at the well head 6.625 diameter casing with a casing head liner visible at surface and 2 production tubing with a tubing head present and pumping rods with the well set up to be produced by the means of a pumping unit with only the tubing head sales line hooked to the well head and feeding into a near by production facility consisting of a single 70 barrel production tank and a 100 barrel production tank. There is a dike present around the production tanks and proper well identification posted.
1502381382 2017-08-10 Production Wells No Violations Status check inspection accompanies by ODNR Inspector Tom Thomas. Upon inspection the well was observed to be equipped with 6.625 diameter production casing with a inner string set inside the production casing as a second production string. The inner string is not visible above the exterior string only the casing well head is. There is a 2.375 diameter tubing string inserted into the inner casing string. The annular space between the inner and outer casing string is open and the well is bubbling gas between the 2 strings slightly. The well is set up to be produced by the means of a pumping unit with both tubing and casing production flow lines present. On location near the well head is 2 production tanks that are not inside and earthen dike area. The production tanks are of steel design and one is and old rivet seam design of approximately 80 barrels capacity and the other tank is a steel 100 barrel capacity tank.
1587274903 2018-03-26 Production Wells No Violations On site inspection accompanied by ODNR Inspector Tom Thomas found the well equipped with 6.625 production casing along with a inner isolation production string and production tubing with pumping rods inserted into the tubing. On locatin near the well head is a production facility consisting of 2 steel design production tanks. Historically Inspector Pat Schreve indicated the inner string set was set as a liner and cemented. The company then unsucessfully attempted to stimulate the well with annular pressure occurring at surface between the inner string and 6.625 production string. The annular of the inner string is open to the atmosphere.
