| 0098176742 | 2016-01-11 | Production Wells | Violations Noted | Inspection request as a response to complaint # 5226 filed by the landowner on 01/05/2015. Upon inspection of the well head the following equipment was found present-6.250 casing -2 production tubing and pumping rods- the well is set up to be produced by the means of a pumping unit with the production valves in the open position. A single 100 barrel production tank is present. The electric meter along with the condition of the well missing the pumping unit gear box indicates no recent production activity, the landowner claims to not be using the gas and indicates the well has not been produced by the current owner. Instructions are as follows-The well owner/operator shall submit a written plugging schedule to Bill Stolarik at 5880 Memory Road Zanesville Ohio 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. ODNR Monroe County Inspector Bill Stolarik 740-507-6592. |

Upon inspection the well was observed to be equipped with only a casing well head visible above grade level with 2 production tubing inserted into the casing head and a 1 plastic sales line hooked to the tubing well head only and no casing line present. The well is set up to be produced by the means of a electrically driven pumping unit with no electric motor present on the unit. There is a total of (2) 100 barrel capacity production tanks present on the property associated with the 5 wells present. Also noted was during the last 1/2 of 2015 a large diameter natural gas line was installed across the property the well is located on and during the installation both the production sales lines and the electrical power supply was disrupted and damaged to the 5 wells on the property. Upon the time of my inspection the flow lines and power service have not been repaired or restored. Photographs taken on this day. |
