Mr. Allphin indicated he submitted registration documentation to the Division regarding the 3 unregistered wells on the property. There was a previous violation indicated as -1128581434 issued and a complaint received by the landowner indicated as 5226. Also on this day I sent a request to the Divisions Surety and Bonding section for conformation that documentation has been received regarding the registration of the wells. |

A total of (2) 100 barrel production tanks are present on the property associated with the 5 wells present. Also noted was during the last 1/2 of 2015 a large diameter natural gas line was installed across the property the well is located on and during the installation both the production sales lines and the electrical power supply was disrupted and damaged to the 5 wells on the property. Upon the time of my inspection the flow lines and power service have not been repaired or restored. Also noted was the well registration is not complete and production has not been reported to the Division. |

Upon inspection of the well head the following equipment was found present-6.250 casing -2 production tubing and pumping rods- the well is set up to be produced by the means of a pumping unit with the production valves in the open position. A single 100 barrel production tank is present. The electric meter along with the condition of the well indicates no recent production activity, the landowner claims to not be using the gas and indicates the well has not been produced by the current owner identified as Genesis Resources -owner # 9086. Instructions are as follows-The well owner/operator shall submit a written plugging schedule to Bill Stolarik at 5880 Memory Road Zanesville Ohio 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. On this day I also received a call from the landowner Mike Christman at 10:34 am indicating there had been a fire in the woods on the previous day, the landowner indicated that an electrical line from one of the wells on the property was the cause. He also indicated the local Fire Department had been on scene. I contacted the producer a Andrew Aallphin with a Genesis Resources and he agreed to meet me on scene on that day. I arrived at 15:00 pm and was met by the landowner Mike Christman and Andrew Aallphin near the scene. Upon inspection it was observed that approximately 1 acre of wood lot surface had burnt, upon inspection it was also observed that at one point both the electric line and 1 plastic flow line associated with the said well had also been damaged. No immediate cause of ignition could be determined. Mr. Aallphin has been provided with the proper well identification for the said wells on the property and asked to properly register the correct wells present as described in the violation previously issued on 01/11/2016. It appears as though the producer had made attempt to put the wells into production prior to the fire. The wells on the property had been out of production due to a large diameter pipeline being installed and the pipeline company damaging both the electrical service and production flow lines associated with the wells on the lease. |
