| 0408545563 | 2017-03-15 | Production Wells | No Violations | Complaint # 5226 and violation # -1281456945 follow up inspection found the following present at the well head 5.187 diameter production casing and 2 diameter production tubing. There is both casing and tubing well heads present with a 1 sales line attached to the casing head and a 1 plastic line attached to the tubing head. The well is set up to be produced by the means of an electrically power driven pumping unit. Also noted was during the last 1/2 of 2015 a large diameter natural gas line was installed across the property the well is located on and during the installation both the production sales lines and the electrical power supply was disrupted and damaged to the 5 wells on the property. Upon the time of my inspection the flow lines and power service have not been repaired or restored. Also noted was the well registration is not complete and production has not been reported to the Division. A total of 2 100 barrel production tanks are present on the property associated with the 5 wells on the property allegedly owned and being produced by Genesis Resources. |

Upon inspection of the well head the following equipment was found present-6.250 casing -2 production tubing and pumping rods- the well is set up to be produced by the means of a pumping unit with the production valves in the open position. A single 100 barrel production tank is present. The electric meter along with the condition of the well indicates no recent production activity, the landowner claims to not be using the gas and indicates the well has not been produced by the current owner identified as Genesis Resources -owner # 9086. Instructions are as follows-The well owner/operator shall submit a written plugging schedule to Bill Stolarik at 5880 Memory Road Zanesville Ohio 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days.
