1302289343 2016-11-18 Plug / Plug Back No Violations 11/18/2016-On site inspection to witness the cement plugging process with a service rig on location owned by Chuck Dye and Formation Cementing. The rig pulled 1,285 of 4.5 from the well and left the 6.25 set to 1,250. The 2 tubing and .625 rods was pulled and the tubing tallied and ran back in to 1,435. A 100 sack bottom plug was pumped with 10 barrels of 6% gel spacer ahead with the well circulating during displacement. The tubing was then pulled up to 935 and a 300 51 sack plug set on top of the 1st plug. A surface plug was then pumped from 300 to surface. The well is owned by DC Oil & Gas Inc. and was improperly identified when permitted and registered with 3411161104, Patty Nicholas in the Divisions Permitting section was contacted on this day and will correct the issue.
