0101679809 2019-01-22 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. Site meets 1509.072 (A) Vegetation is established with no substantial erosion. I found the pad gated with signage at the entrance. There are (4) producing wells, (4) gas processing units (GPU), (4) 400-barrel and (1) 100-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads and the GPUs. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 300 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. Bubbling was witnessed within the cellar, and a CNX operator was notified. The issue will be assessed.
0728615822 2018-08-02 Drill / Deepen / Reopen No Violations On this day, I found the Patterson 803 drilling rig tripping back in the hole after cementing the 9.625 inch diameter deep intermediate casing. I spoke with Company Man, Dewayne Finney and Engineer Alex McCutcheon during my inspection. The most current survey taken at a measured depth of 9,975 feet (TD of 12.25 inch diameter borehole) had an azimuth of 313.99 degrees and an inclination of 14.1 degrees. They will commence drilling in their 8.75 inch diameter production borehole once on bottom. The projected total depth of the well is 22,562 feet measured depth, and Mr. Finney indicated that 6 inch diameter production casing will be run on this well. A pad inspection was conducted with no noticeable issues.

1522257520 2018-03-28 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling process found Decker Drilling rig # 8 on location and currently rigging up to drill out from under the 18.625 mine string. I arrived at 10:00 am and found the rig attempting to rig up their air body in preparation of drilling their 17.5 diameter production borehole. The recent 18.625 diameter mine sting cement job was reviewed and it was determined that the company achieved proper circulation during the cement job and no mine void was encountered. There was a swell packer and stage tool ran in the 18.625 diameter mine string above the coal seam that came in between the depths of 713 and 724.
1522432899 2018-03-22 Casing String Witnessed No Violations The purpose of my inspection was to witness cement operations on the conductor casing. Express Energy was on location with 2 auger rigs, 1 bulk cement truck, and one pump truck. The borehole was 32 inches in diameter and was drilled to 100 feet in depth. A 26 inch diameter conductor was set on bottom of the borehole at 100 feet. Express held a pre job safety meeting with all personnel on location. Following the safety meeting Expressed plumbed up to the casing and pumped 10 barrels of fresh water ahead of 240 sacks of class A cement weighing 15.6 pounds per gallon and 55.7 barrels of fresh water for displacement. 10 feet of cement was left within the casing and cement circulated to surface.
