1522685029 2018-04-02 Casing String Witnessed No Violations On site inspection at 5:00 am to witness the surface-mine string cement job with Decker Drilling rig # 8 on location along with Halliburton Cement Service. The rig has drilled a 24 diameter surface-mine string borehole to a measured depth of 800 with the depth measured off a drilling rig Kelly Bushing heigth of 12 above ground level, There was a coal seam encountered between the depths of 713 and 724. Proceeding the drilling the drill string was pulled and 762 of 18.625 diameter J-55 Grade buttress thread casing was ran to a depth of 762 with a casing shoe ran on bottom and a swell packer and DV tool set in the string between the depths of 457 and 469. Halliburton cemented the casing in a 2 stage cement job with the bottom section below the mine cemented using a 65 barrel cement mix consisting of 400 sacks of class A cement with the cement weight verified t 15.8 pounds per gallon by weight of cement. The 1st stage was displaced with 233 barrels of fresh water and upon the landing of the cement plug the pressure was increased to 500 psi to verify casing integrity. Following a 5 minute casing test the pressure was increased in increments slowly until the swell packer deployed and inflated at 1,020 psi. Once the packer was confirmed as deployed the DV tool sleeve opening tool dropped the 2ns stage cement job commenced. The upper section was cemented with a 130 barrel class A cement mix consisting of 600 sacks of cement weighing 15.8 pounds per gallon by weight of scale. The cement was displaced with 143 barrels of fresh water with 34 barrels of cement returns witnessed to surface. The displacement pressure was 190 (PSI) pounds per square inch upon the landing of the plug and increased to 883 psi to verify casing integrity.
1528816149 2018-06-12 Drill / Deepen / Reopen No Violations On this day, I found the Patterson 803 rig is currently drilling the deep intermediate 12.25 inch diameter borehole at a depth of 4,100 feet. The most recent survey had an inclination of 22.02 degrees and an azimuth of 286 degrees. The projected total depth for the 12.25 inch borehole is 10,187 feet. I spoke with Company Man, Dewayne Finney during the inspection.

1727268288 2019-01-22 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. Site meets 1509.072 (A) Vegetation is established with no substantial erosion. I found the pad gated with signage at the entrance. There are (4) producing wells, (4) gas processing units (GPU), (4) 400-barrel and (1) 100-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads and the GPUs. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 1,000 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 200 psi.

2006644483 2018-10-09 Completion Testing No Violations Status check inspection found 4 wells present on the pad and the well drilling process complete on all 4 wells. Keane Pressure pumping Service is on location and 2 of the 4 wells present have had the stimulation process complete. The said well is complete with 47 stages being stimulated with the last initial shut in pressure being 6,852 pounds pressure before the isolation plug was set. All annular pressures are monitored electronically during stimulation and while the remainder of the wells on the pad are being stimulated.
