0063548554 2018-06-25 Completion Testing No Violations Status check inspection on the well completion process found the company has installed a lower master valve and is preparing the well for stimulation. Annular pressure monitoring is in place and is being checked daily. The company will fill the mouse holes and pour extra concrete in the cellar up to the base plate of the surface casing tomorrow morning. This will be to add extra support to the well head. The well pad earthen poly liner is intact with rubber mats in place on the liner. All rig equipment is off site. There will be an above ground storage tank placed on the well pad location.
0330093791 2018-08-21 Completion Testing No Violations Status check inspection found the well drilling -stimulation and completion drill out operations now complete. The company man on site indicated that 2.875 diameter L-80 grade 6.5 pound per foot production tubing was set in the well bore on a tubing hanger. The tubing was set to a measured depth of 10,141 with a tubing profile X nipple set in the string at 9,810 and an XN nipple also set in the string at 10,105. The tubing hanger was positive pressure tested to 7,500 pounds per square inch for 15 minutes then negative tested for 15 minutes also. The well head consists of a lower A and upper B section followed by a production flow cross then a tubing flow cross. The tubing flow cross consists of a primary master valve followed by a secondary master valve with a total of (2) emergency shut down valves present in the well head production assembly. Crews are on site installing the production equipment downstream from the well head assembly. Crews are also on site mopping up from the completion process which includes pressure washing and removing all containment mats, all fluid created is being recovered and hauled from site by the means of a vacuum truck. All perimeter drain valves are in the closed position and no surface water is being discharged. 3 wells are present on this pad.
0565492450 2018-04-13 Drill / Deepen / Reopen No Violations Status check inspection to document the current well drilling status. H&P Drilling rig # 371 is on location and currently drilling the # 2-H well vertical section. The said well the Pool # 3-H has had the conductor- the 17.5 diameter surface casing and the 9.625 1st intermediate casing string set. The 9.625 diameter 43.5 pound per foot casing was set down to a measured depth of 9,201 and cemented back to surface by C&J Cement Service with the 9.625 diameter casing cement ticket reflecting a total of 228 barrels of cement returns to surface. The company men on site have been to directed to monitor all casing annular pressures during the well drilling process as indicated in 1501:9-1 of the well construction rules.
0611025343 2018-03-22 Casing String Witnessed No Violations The purpose of my inspection was to witness cement operations on the conductor casing. Express Energy was on location with 2 auger rigs, 2 bulk cement trucks, and one pump truck. The borehole was 26 inches in diameter and was drilled to 120 feet in depth. No water was encountered during the drilling process. A 20 inch diameter conductor was set on bottom of the borehole at 120 feet. Express held a pre job safety meeting with all personnel on location. Following the safety meeting Expressed plumbed up to the casing and pumped 10 barrels of fresh water ahead of 230 sacks of class A cement weighing 15.6 pounds per gallon and 39 barrels of fresh water for displacement. 10 feet of cement was left within the casing and the remaining cement filled the annuals and circulated to surface. Returns were siphoned from the cellar.
0851800895 2018-04-04 Drill / Deepen / Reopen No Violations On this day, Im visiting this wellsite per request of ODNR Inspector William Stolarik. I found the H&P Flexrig 371 rig is currently drilling the 12.25 inch diameter wellbore at a depth of 6,163 feet. I spoke with Company Man Tim Fields and he indicated that they plan to batch-drill three wells through the intermediate casing string, and skid the rig back to complete the lateral section. Due to a recent rain event, the pad became flooded with rainwater, so it was decided to have an environmental group, K.C. Harvey, test their water, and then drain the pad, prior to my visit. The valves have since been closed, and vac trucks will be onsite to remove the residual water. It will also be noted that the well is currently being drilled on air, and experiencing occasional, non-sustained flares. No flare was witnessed during the time of inspection. Tim Fields notified myself, and additional contacts of the flare, as required by their permit condition. I also advised to continue maintaining filter socks along the slopes of the wellpad, after the rain event.
