0056398978 2018-04-09 Casing String Witnessed No Violations Onsite inspection to witness the hydrostatic test, after completing the 18.625 inch diameter surface mine-string casing. The mine-string had previously parted and fell downhole and was recemented. CNX was given permission to proceed with their operations by ODNR. The Patterson 803 rig has drilled a 17.5 inch diameter borehole through the cement shoe, to a depth of 1,085 feet. The hole is loaded with 10.3 pounds per gallon brine to perform the hydrostatic test. A 45-minute hydrostatic test was successfully completed.
0110235525 2019-04-08 Production Wells No Violations On this day, I am accompanying ODNR Engineers Mark Atkinson and Andy Thomas, ODNR Southeast Supervisors Brent Bear and Mike Moreland, and CNX Gas Representatives John Hardcastle, Rick Harless, and others. The meeting is to review the new proposed well locations. The existing production wells were also inspected. The 9.625 inch diameter intermediate casing has a pressure of 980 pounds per square inch (PSI), and the 13.375 inch diameter surface casing has a pressure of 500 PSI. On 4/8/2019 I left a message with John Zachwieja, CNX Production Foreman, to notify him of the pressures.
0730816944 2019-01-22 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. Site meets 1509.072 (A) Vegetation is established with no substantial erosion. I found the pad gated with signage at the entrance. There are (4) producing wells, (4) gas processing units (GPU), (4) 400-barrel and (1) 100-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads and the GPUs. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 900 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 400 psi.
1522866859 2018-04-04 Casing String Witnessed No Violations On site inspection follow up to a failed 18.625 casing cement job that took place on 04/03/2018. My inspection found Patterson Drilling rig 803 on location and rigged up on the hole. The rig had drilled a 24 diameter surface borehole to a measured depth of 1,000 with a open mine void encountered between the depths of 843 and 857. Initially the well bore was drilled and casing was attempted to be ran on 04/01/2018 unsuccessfully due to not being able to get the casing more than 19 past the mine floor. The company pulled the casing and had to ream the well bore before successfully installing the 898 of 18.625 diameter J-55 buttress thread casing on 04/03/2018. Installed in the casing string was a Davis Lynch stage tool and swell packer set between the measured depths of 594 and 597 with all measured depths taken from a drilling rig Kelly Bushing heigth of 29 above ground level. On 04/03/2018 during the cementing process the stage tool was being inflated between the 1st and second cement stages when the casing jumped aggressively upward severing the tie down chains attached between the cement head and rig floor. One of the chains struck a Patterson Drilling employee critically injuring Him. At that point work was stopped until CNX and Patterson conducted their investigations. On this day I reviewed all documents and witnessed KLX Energy run a camera survey log to attempt to verify the integrity of the well bore 18.625 diameter casing string. The casing was landed into a GE Company well head mandrill system. I was on location from 9:00 am and 14:30 pm and witnessed the camera log being ran. I left location while the log was still being ran with no conclusive determination at the time of my departure of the integrity of the casing.
1522945182 2018-04-05 Casing String Witnessed No Violations Status check inspection to witness the company grout the annular side of the 18.625 diameter casing string. On the previous day I was on site to witness a camera log being ran. The company reported later in the day after I left location that they believed the casing had parted 52.09 feet above the DV tool set at 594. The company also indicated they believed the lower section of the casing string fell downhole 30 and the DV tool was sitting at 624. I requested the company submit a plan in writing and on the evening of 04/04/2018 a plan was received from a Gregory Zofchak with CNX. I received the plan and submitted it over to my Regional Supervisor Mike Moreland as well as District Supervisor Brent Bear and Deputy Chief Dave Claus. The plan was reviewed by Managers Bear and Moreland and permission was given to proceed. Following being granted approval to proceed the company pulled the upper section of the 18.625 diameter casing string out of the mandrill well head and ran in the hole until the tagged the lower casing string and at that point indicated they were able to successfully screw both string sections back together with 18,000 pounds torque pressure achieved. The company then ran a 1 diameter tubing string into the well to a depth of 520 from the rig floor KB heigth of 29 above grade level. At that point the well bore was grouted by Halliburton with a first attempt of 155 barrels of Class A cement weighing 15.6 pounds per gallon and a yield of 1.179. No cement returns were witnessed on this first attempt.
1523233872 2018-04-03 Casing String Witnessed No Violations The purpose of my inspection was to witness 902 feet of 18.625 inch diameter surface casing. A coal seam was from 843 feet to 859 feet in depth. The job will be pumped in two stages with a differential valve tool and an inflatable packer. Halliburton was on location to complete cementing operations. During my inspection the first stage of the cement job was pumped and the packer was inflated. While inflating the packer the casing broke the chains off of the cement head. The cement job was discontinued at this point.
