0003452869 2017-08-01 Casing String Witnessed No Violations The purpose of my inspection was to witness 100 feet of the 30 inch diameter conductor being cemented in a 36 inch diameter borehole. Express Energy drilled the borehole and did not encounter any water during the drilling process. A pre-job safety meeting was held with all personnel on location including Express Energy, CNX Company Man, and Wise Trucking. Following the safety meeting Express Energy pumped 10 barrels of fresh water, 275 sacks of class A cement with 2% gel, and 75 barrels of fresh water. 10 feet of cement was left inside the bottom of the conductor and 20 barrels of cement returned to surface.

1427650028 2018-01-18 Completion Testing No Violations Status check inspection on the well completion process found a service rig rigged up on the well owned by CWS Services. The rig has drilled out all of the 42 stages of completion plugs and is installing the 2.875 diameter production tubing in the well to be set at a measured depth of 10,407. This is the last of 3 wells on the pad to be completed and the production tubing installed. The well is slated to go on line and put into production on 01/24/2018. Rock Water Flow back service is on location and will monitor the initial flow back stages.
1834080580 2019-04-25 Production Wells No Violations On this day, I found (3) production wells surrounded by steel cages in preparation for simultaneous operations to take place during the drilling process of (2) additional wells. Digital pressure monitoring devices are equipped on each well. The pressure on the 9.625 inch diameter casing is 454 pounds per square inch (psi), and the 13.325 inch diameter casing is 448 psi.
1991091715 2019-01-22 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. Site meets 1509.072 (A) Vegetation is established with no substantial erosion. I found the pad gated with signage at the entrance. There are (3) producing wells, (3) gas processing units (GPU), (3) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads and the GPUs. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 1,000 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 1,000 psi. It will be noted that maintenance is be conducted on the wellheads. I spoke with John Zachwieja, CNX Production Foreman, during the inspection and he stated that they are currently replacing the choke valves in the wellheads. Bubbling was witnessed within the cellar, and John indicated that a vac truck will be on location to suck out the cellar, and assess the issue. Also, Certarus, a compressed natural gas provider, is located onsite compressing natural gas after being metered by CNX. John estimated that they will be onsite until late spring.
