0668999842 2019-01-22 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. Site meets 1509.072 (A) Vegetation is established with no substantial erosion. I found the pad gated with signage at the entrance. There are (3) producing wells, (3) gas processing units (GPU), (3) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads and the GPUs. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 950 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
1495738224 2017-05-25 Casing String Witnessed No Violations On site inspection to witness the conductor casing cement job with Express Energys top hole rig and cement service. The auger rig has drilled a 36 diameter borehole to a measured depth of 100 and inserted 100 of 30 diameter .375 wall 118 pound per foot conductor. Express then cemented the casing with a 48.9 barrel cement mix consisting of 275 sacks of class A cement with the cement weight verified by weight of scale at 15.6 pounds per gallon. Cement returns were witnessed to surface inside the cellar at 50 barrels into a total of 75 barrels of fresh water dissplaceent.1 of cement was left in the cellar causing a liquid tight seal. No fresh water was encountered in the borehole while drilling.
1499096847 2017-06-30 Casing String Witnessed No Violations During my inspection I witnessed Patterson Drilling Rig 803 land the 18.625 inch diameter casing at a depth of 651 feet within a 24 inch diameter hole. BJ services arrived on location to cement the casing string in two stages with a packer set at a depth of 399 feet. A coal seam was encountered during the drilling process from 596 feet to 604 feet. Cement calculations were discussed with the CNX company man. BJ services rigged up and held a pre-job safety meeting with all personnel on location. I was unable to be present for the total duration of the job.
1576112886 2018-01-18 Completion Testing No Violations Status check inspection on the well completion process found the well has been stimulated in a total of 57 stages. The production well head assembly is installed and Deep Well Service tubing snubbing unit # 9 is on location completing a adjoining well. The well support production equipment is present on location consisting of a gas processing unit and production tank farm. 3 wells are total on location.

2123698199 2018-02-04 Completion Testing No Violations On site inspection found the well drilling and stimulation process is complete. The completion plugs have been drilled out and production tubing hung in the well. TETRA flow back services are on site and rigged up to the said well, they will begin the initial well bore production flow back on this day.
