0429444591 2019-05-24 Drill / Deepen / Reopen No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. On 5/22/17 the conductor was cemented in place, but the well was never drilled. It will be noted that ProPipeline is currently working on the northwest slope of the pad, and have removed a section of the fencing and berm. ODNR Engineering Technician Andrew Thomas will be notified. The preliminary restoration will be addressed at a later date.

1880904405 2017-05-22 Casing String Witnessed No Violations On site inspection to witness the conductor casing cement job with Raymond Drilling top hole rig and O-Tex Cement Service. The well was spudded on 05/22/2017 and a 36 diameter conductor borehole drilled to solid bedrock at 94. The rig then installed 94 of 26 diameter .375 wall casing with weld connections. O-Tex Cemented the casing using a 81.9 barrel cement mix consisting of 380 sacks of class H cement. The cement weight was verified at 15.6 pound per gallon by weight of scale. The cement was displaced to 77 with 48 barrels of fresh water with cement returns witnessed to surface.
