0568184954 2019-05-16 Completion Testing No Violations On this day, I found the drilling and hydraulic fracturing processes are completed. The frac plugs have been drilled out and the well is currently in the flowback stage. I spoke with Gulfport Representatives Corey Cloud and Robert Coots during the inspection. Annular pressure monitoring devices are in place and pressures are logged daily. Identification is posted at the wellhead.
1025013760 2017-02-01 Drill / Deepen / Reopen No Violations On the day inspection I witnessed that Lightning Energy Services had drilled a 36 inch diameter hole to the depth of 110 feet, hitting water at 40 feet and ran 110 feet of 26 inch diameter 85.6 pounds per foot conductor pipe. Stingray Pressure Pumping was onsite pumped 2 barrels of fresh water, performed a 500 pounds per square inch pressure test, gained circulation with 10 barrels of fresh water, pumped 15 barrels of gel, pumped 10 barrels of fresh water and cemented pipe with 470 sacks Class A cement with 2% CaCl and .25 pound per sack of celluseal weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 100 feet with 60 barrels of fresh water with good cement returns of 19 barrels to surface. Mix water was tested revealing chlorides of 43 parts per million, pH 7 and 53 degrees.
1493111241 2017-04-25 Casing String Witnessed No Violations On site inspection to witness the Surface casing cement job accompanied by ODNR Inspector Andrew Corder. Pioneer Drilling rig # 73 is on location along with O-Tex cement service. The rig has drilled at 17.5 diameter surface borehole to a measured depth of 1,385. The rig then proceeded to pull the drill string and run the 54.5 pound per foot J-55 grade buttress thread casing with the last 3 casing joint circulated in the hole to bottom. O-Tex Cement Service cemented the casing by first pumping 50 barrels of 6% gel spacer followed by 262 barrels of cement consisting of 1,220 sacks of class H cement with 3% calcium and 1/4 pound per sack loss circulation material. The cement weight was verified at 15.6 pound per gallon by weight of scale. The cement was displaced to the float collar with a total of 207 barrels of recycled water, cement returns were witnessed to surface at 165 barrels into displacement. The casing was pressure tested to 1,150 PSI with a displacement pressure of 629 psi prior to the landing of the plug.

1529328663 2018-06-18 Completion Testing No Violations On this day, I am conducting an inspection per request of ODNR Inspector William Stolarik. I spoke with Company Man, Ken Miller, during my inspection. (10) wells were drilled on the pad. They are currently plumbing the intermediate and surface casing, at the wellhead, and preparing to install pressure monitoring devices. The grates will be replaced over the cellar, when they are finished.
1537802305 2018-09-24 Completion Testing No Violations Status update inspection found a total of 10 wells present on this well pad. All 10 have been drilled and not completed. Each well is equipped with a lower A and upper well head section with 2 diameter riser pipes present on each section with valves and gauges present on each section for annular pressure monitoring. The casing head does have a multi valve set up with a mechanical gauge present. No tubing is in the well. No production equipment is present on the well pad other than the 3rd party mid stream pipeline. The well pad does have an earthen berm in place. The well pad surface and lease road is of stone construction and in good repair. Addressing the preliminary restoration. There is an area along the lease road that is within the well site boundary that was recently disturbed by the installation of a transportation pipeline. There is also a area on the West side of the well pad that was disturbed and vegetation not established. Larger diameter wooden poles are set on the East side of the well pad along the pad surface boundary for sound barrier during stimulation. There are (2) 12 diameter trunk lines that have been laid along the well pad perimeter, these lines are fresh water lines to support the stimulation process. The lease road does have a gate present.
2100637659 2019-02-13 Completion Testing No Violations On this day, I am conducting an inspection during the completion process, and found Halliburton Pressure Pumping Service onsite along with Reach Wireline. They are zipper fracing between permit numbers 4712 (3B) and 4715 (1B). I spoke with Gulfport Company Man Dave Cole during the inspection. They just completed stage 1 of 49 with 28 shots, divided into 4 clusters. The frac plug for stage 2 will be set at a measured depth of 19,373 feet. Mr. Cole stated that annular pressures for the 13.375 inch diameter casing and 9.625 inch diameter casing are monitored and logged daily for all (10) wells on the pad. He also stated that only 5 of 10 wells will be completed at this time.
