0148403812 2017-02-07 Casing String Witnessed No Violations The purpose of my inspection today is to witness the surface-mine string cement job. Patterson Drilling Rig 801 drilled a 24 inch diameter borehole to a depth of 682 feet. The mine zone is from 575 feet to 600 feet in depth. The 18.625 inch diameter casing was run to a depth of 643 feet. Allied Cement Services is on location to cement this casing string in two stages with a packer set at 343 feet. 150 barrels of fresh water, 50 barrels of gel, and 10 barrels of fresh water were pumped ahead of the cement. 50 barrels of Class A cement with 1.5 percent calcium and 197 barrels of displacement were pumped to complete the first stage. The plug landed and the packer was energized. The second stage was completed using 88 barrels of fresh water broke circulation, 25 barrels of gel, 10 barrels of fresh water, and 168 barrels of Class A cement with 1.5 percent calcium. 106 barrels of displacement were pumped and an estimated 65 barrels of cement returns were witnessed in the cellar.
0702576189 2017-02-27 Casing String Witnessed No Violations During my inspection Patterson Drilling Rig 801 ran the 5.5 inch diameter production casing to 21,035 feet within a 8.75 inch diameter borehole with a total depth of 21,065 feet. CNX company man stated that they will circulate to clean the hole and cement the casing string tonight.

1514398859 2017-12-27 Work Over Reconditioning No Violations Status check inspection found a workover tubing snubbing unit rigged up on the well and fishing a lost 2.875diameter workover string. The string became stuck inside the 5.5 diameter production casing string on 08-12-2017 when a hole was created in the string near surface and the company was forced to shear the tubing off with the blow out preventer blind shears at surface. The tubing has been recovered to measured depth from surface of 11,568 with the bottom of the workover string at 16,001, the string has a workover drill bit and motor still attached also. The well head surface control blow out preventer consists of a KLX and Wildcat combination stack assembly with the Deep Well Snubbing unit sitting on top of the workover blow out preventer. Cudd Service coil tubing unit is standing by to make a clean out run on the lost tubing string inside diameter so recovery efforts may proceed. Tetra flow back service is providing flow back to the said well and also to the other 3 wells on the pad that is complete and in production.
1533572396 2018-08-06 Preliminary Restoration No Violations Preliminary restoration status check inspection found the well site meets ORC 1509.072 with vegetation established and erosion control in the area affected by the well pad construction. The well pad surface and lease road has a stone surface and is in good repair. There is a EQT midstream transmission point on the right rear corner of the well pad. Fire extinguishers and emergency shut down well control devices are situated at all critical areas of the well pad along with the well pad muster points. 4 wells are present on the pad. The said well did have tubing lost in the well bore during the worker completion of the well. The tubing was not recovered and the well is currently shut in and not producing, however there is pressure at surface on the well head. The well head consists of a lower A and upper B section along with a production tree. Present at surface inside the cellar and attached to the A & B section are riser pipes and valves with mechanical and electronic gauges present. The A section pressure gauge which is the 13.375 diameter casing string internal pressure between the 13.375 and 9.625 casing strings showed 695 pounds per square inch (psi). The B section valve which is the internal pressure on the 9.625 diameter casing (between the 9.625 and 5.5) showed 1,014 psi. The 5.5 diameter casing internal pressure gauge read 4,534 psi and the tubing head pressure gauge showed 4,634 psi. The well head was shut in. Immediately on the right upon entering the well pad is the production facility consisting of 4 steel construction 400 fluid barrel capacity storage tanks all situated inside a polymer lined steel containment dike area.
2084062215 2018-06-20 Production Wells No Violations Status check inspection found there is 4 producing wells on the pad. All wells have had the drilling and completion process complete and are in production. The well are producing off the production tree with emergency shut down devices in line. Annular pressure monitoring is in place with 1,068 psi showing between the 5.5 and 9.625 and 622 psi showing between the 9.625 and 13.375. On site is 4 gas processing units and a production facility consisting of 4 steel tanks capable of holding 400 barrels capacity each. The tanks are situated inside a polymer lined steel containment dike area. . I spoke with Consol regarding clarification on the pressure readings on the well head annulis and filling in the drilling rig mouse holes. There also was some house keeping issues addressed.
