0236376013 2018-08-06 Production Wells No Violations Status check inspection found 4 wells producing on the pad. The pad surface is of stone construction with a earthen perimeter berm, the perimeter valves are closed. The well head is equipped with a lower A and upper B section along with a production tree. The well is currently in production with the production valves open. The well head has riser pipes and valves with gauges present to monitor annular pressure on the A & B sections with 275 pounds per square inch pressure (psi) on the A section which monitors the pressure on the 13.375 diameter casing string (between the 13.375 and 9.625). The pressure on the B section which monitors the pressure on the 9.625 casing string (between the 9.625 and 5.5 diameter casing strings) is 872 pounds pressure. The 5.5 diameter production casing internal working pressure is 1,653 psi. The production flowline pressure is 1,000 psi. The well head assembly is equipped with 2 manual and one emergency shut down valve on the vertical section of the production tree followed by a single manual valve on the horizontal outlet section of the production tree. Also on location is 4 gas processing units along with a production facility tank farm consisting of 4 steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. Proper well identification is posted at the well heads along with hazard and identification on the tank battery. At all critical hazard and muster points of the pad are fire extinguishers and emergency shut down master well control devices. A EQT midstream point is on the right front corner of the well pad.
1182673358 2016-09-03 Casing String Witnessed No Violations On site to witness running and cementing of 13.375 surface casing they drilled 17.5 hole to 2400 and ran 2372 of new casing and cemented it in place with 1791 sacks of cement and they had 60 bbl. of good returns and casing was equipped with guide shoe and float collar and 21 centralizers.
1254225231 2016-08-13 Casing String Witnessed No Violations I am onsite to witness the conductor installation and cementing operations. Express Energy Services utilized an auger drilling rig to drill a 36 borehole to a depth of 100. Express then installed 100 of 30 .375 wall weld casing. Finally, Express cemented the casing in place with 275 sacks of class A cement with .25 pounds per gallon of flowseal. The cement was verified by scale at 15.7 pounds per gallon. I witnessed 15 barrels of cement circulate to surface. Six inches of cement was left in the cellar to provide a liquid tight seal at the base. While drilling at 47 a moderate flow of fresh water was encountered and remained through the job.

1473868997 2016-09-14 Drill / Deepen / Reopen No Violations Status check inspection accompanied by Inspector Austin Gutridge found Patterson Rig 803 is on location and is currently drilling the 8.750 production bore out from under the 9.625 casing set at 9,453. A formation integrity test was administered below the 9.625 with a 15 ppg equivalent achieved at 1,224 psi. The rig is pulling the drill string to replace a failed direction tool .

1514915506 2018-01-02 Production Wells No Violations On site inspection as to a response to a report of a 25 gallon fluid release on the EQT midstream production pad that is situated on a site pad that adjoins the main well pad. The pad is situated on the South West side of the Switz 5 well pad and was deplicted on the construction submittal plans in 2015. No product was visible on the earths surface and there was a roll off container on site with material place in it that had been recovered. Addressing the well, the drilling and completion process is now complete and the well is in prodcution along with 3 additional wells on the pad. There is a total of (4) gas processing units on the well pad that the wells produce into. Alos present is a production fluid storage facility consisting of (4) steel concruction 400 barrel capacity production tanks situated inside a polymer lined steel containment dike atrea. Proper identification is posted at multiple locations on site.
1650027348 2016-08-31 Casing String Witnessed No Violations On site to witness running and cementing of mine string on said well, hole was drilled to a depth of 741 and they ran 719 of 18.625 casing equipped with guide shoe and float collar and a TAM packer system that was set above suspected mine area and cement stage tool above packer and also two cementing baskets and nine centralizers and ALLIED Cementing pumped 1404 sacks of Class A CEMENT WITH 50 BBL OF GOOD CEMENT RETURNS.
