0210020812 2017-01-05 Production Wells No Violations On the day of inspection accompanied by ODNR Senior Health Physicist Paul Carder, I witnessed the well was online with the valves in the open position. Gauges present read 6995 pounds per square inches on the casing and 6100 pounds per square inches on the tubing with annular monitoring in place. Production equipment included a sand trap, a line heater and a 2- stage vertical separator. Also present included 4-400 barrel tanks within containment, 2 filter separators, 2 vertical scrubbers, 2 heater treaters and a gas combustor all shared with 34111246100000 Pittman 3H, 34111245920000 Holliday A 1H, 34111245960000 Holliday B 7H and 34111246090000 Circosta 1H. We met with Eclipse EHS Technician Ken Durst and completed walk around revealing containment was being removed. I instructed Mr. Durst to fill the mouse holes in accordance with OAC 1501:9-11-11 and to continue to pull fluids out of the sump until the levels are acceptable.
0531703336 2016-11-29 Completion Testing No Violations On the day of my inspection I witnessed Deep Well Service Stand Alone Snubbing Unit # 9 was tripping into the wellbore to drill out the kill plug set at 10048 feet with a 4.5 inch diameter tricone bit. Tetra Services was onsite controlling well pressure along with flowback of fluids.
0674127184 2016-11-22 Administrative Inspection No Violations I received a call from Eclipse EHS Supervisor Bruce Carpenter at 0940 stating that a truck hauling tubing to location Kender Hauling had drove from Clarington Ohio with a fuel leak following the RUMA route. The truck pulled onto the access road releasing less than 5 gallons of diesel fuel before containment was placed under the fuel tank. The truck was repaired and HTP was placed on the access road where he leak occurred and removed with a skid steer for disposal.

I met with Consultant Cristo Giannareas and completed walk around.
1439734416 2015-08-16 Casing String Witnessed No Violations The said well was spudded on 08/14/2015 by Precision Rat Hole with a 30 x 120 conductor borehole being drilled and 120 of 24 .375 wall 94.62# pound per foot weld connection casing being ran. Universal Well Service is the cement contractor on location to cement the casing using 320 sacks of class A-3% calcium .25 pound per sack loss circulation material with the pipe capacity being displaced with 53 barrels of fresh water and 10 barrels of 6% gel prior to cementing and good cement returns witnessed to surface.
1442119122 2015-09-13 Casing String Witnessed No Violations On site inspection to witness the surface casing installation and cement job found the following -H&P rig # 531 is now on the hole and has drilled a 1,800 x 17.5 surface borehole and ran 1,743 of 13.375 54.5# casing with a casing guide shoe and float collar ran and proper centralizer placement demonstrated. Schlumberger Cement Service is on location to cement the casing using 1455 sacks of class A- 25% calculated open hole excess cement with 20 barrels of 6% gel spacer ran ahead of the cement and circulation established to surface prior to cementing. The casing was pressure tested to 500 psi over lift pressure and good cement returns were witnessed to surface.

1443461756 2015-09-28 Casing String Witnessed No Violations On site inspection to witness the 5.5 production casing cement job. The said well was drilled to a total measured depth of 20,789 on 09/25/2015 by H&P Drilling rig 531 with 20,777 of 23# lb/ft 5.5 casing ran with a float collar and guide shoe casing equipment ran along with proper centralizer placement demonstrated, also noted was a total of (4) Nine energy flow guns ran between the depths of 20,731 and 20,473. Schlumberger Cement Service was on location to cement the casing using a 1870 sack lead 50/50 poz class A 14.2# ppg cement mix and 2,450 sack 50/50 poz class A 14.8# ppg tail cement mix with 50 barrels of 13.8# ppg Mud push sweep ran ahead of the cement. There was 3,900 psi during displacement @ 3.9 barrels per minute with good circulation to surface and spacer returns at 400 barrels into displacement with a calculated total of 439 to bump the plug, the company over displaced to 449 total barrels with no plug seating encountered at 5,900 psi total encounter pressure with a 6,500 maximum allowed pressure due to the Nine Energy Flow guns. The pre job design calculated top of cement was 1,000 measured depth. No further evaluation is to be required due to adequate lift pressure and fluid returns through out the cement job.

2010374267 2018-05-22 Production Wells No Violations Status check inspection found the well drilling and completion process now complete and the well producing through 2.875 production tubing with 2 manual and 2 emergency shut down in line valves present in the production flowline. Proper well identification is posted. Annular pressure monitoring is in place on the 13.375 and 9.625 well head sections with 2,300 psi on the 9.625 and 0 psi on the 13.375 well head sections. Production related equipment on site consists of 5 gas processing in line heaters and 5 sand trap separators along with a single glycol heater. The production fluid facility consists of 4 steel construction tanks of 400 barrel capacity situated inside a polymer lined steel containment area.
2046048720 2017-03-21 Preliminary Restoration No Violations Preliminary Restoration found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is in good repair and stoned. The location entry point is posted with the owners contact information. The location is fenced with chain link fence. There is a total of 6 wells on the pad 5 of which are complete and all 6 have identification posted. The said well has a casing and tubing well head in place and is equipped with tubing head valves and B section cross valves. There is a Hydraulic operated electrically controlled emergency valve on the tubing head output flow line that has shut down points at the well head and production facility along with at the well pad entry point. Also on location downstream from the well head is a sand trap and line heater and then a mechanical separator. The mechanical separator sends the processed fluid to the production facility that consists of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment area. There is also 2 dehydration units on location that service the 6 total line heaters on the pad.

Deep Well Service Stand Alone Snubbing Unit # 9 was drilling out frac plug # 7at 11600 feet with a 4.5 inch diameter tricone bit. Tetra Services was onsite controlling well pressure at 3800 pounds square inch along with flowback of fluids.
2144778108 2016-12-13 Completion Testing No Violations On the day of my inspection I witnessed flowback operations began this morning through Tetra Services flowback equipment. I met with Consultant Cristo Giannareas and completed walk around revealing the well was flowing back with 6500 pounds per square inches on the casing at 18.7 MMCF and 40 barrels per hour through a 24/64 inch choke.
