1065614779 2016-11-21 Drill / Deepen / Reopen No Violations During my inspection the Patterson 803 rig was drilling the 17.5 inch diameter borehole at a depth of 2124 feet.
1479598257 2016-11-19 Casing String Witnessed No Violations On site inspection to witness the surface-mine string cement job with Patterson Drilling rig 803 on the hole along with Allied Cement services and a Black Hawk casing tool representative. A 24 borehole was drilled to 741 and 706 of 18.625 casing was ran with the mine seam being encountered between 665 and 676. Allied Cement Service cemented the casing in 2 stages with the bottom section cemented using 50 bbl (234 Sacks) of class A-1.5% calcium cement with 10 barrels of 6% gel spacer pumped ahead of the cement along with 200 barrels of fresh water ahead to clear the casing. Upon the landing of the bottom cement plug the internal casing pressure was increased to 810 psi to energize the packer, upon confirming the packer was properly inflated the 2 stage tool was dropped and the upper 18.625 casing cement job executed using 156 barrels (730 sacks) of class A-1.5% calcium cement with 25 barrels of 6% gel spacer pumped ahead after circulation had been established to surface. The upper section wiper plug was landed after 144 barrels of displacement and the casing pressure tested to 500 psi over displacement pressure with cement returns witnessed to surface inside the cellar.
1480339467 2016-11-28 Drill / Deepen / Reopen No Violations During my inspection the Patterson 803 Drilling Rig just reached a depth of 9610 feet within the 12.25 inch diameter borehole. The rig will now prepare to run the 9.625 inch diameter intermediate casing.
1509511234 2017-07-18 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with vegetation and erosion control established in the affected area of the well pad. Addressing the well status-
1533580359 2018-08-06 Production Wells No Violations Status check inspection found 4 wells producing on the pad. The pad surface is of stone construction with a earthen perimeter berm, the perimeter valves are closed. The well head is equipped with a lower A and upper B section along with a production tree. The well is currently in production with the production valves open. The well head has riser pipes and valves with gauges present to monitor annular pressure on the A & B sections with 870 pounds per square inch pressure (psi) on the A section which monitors the pressure on the 13.375 diameter casing string (between the 13.375 and 9.625). The pressure on the B section which monitors the pressure on the 9.625 casing string (between the 9.625 and 5.5 diameter casing strings) is 720 pounds psi. The 5.5 diameter production casing internal working pressure is 2,060 psi. The production flowline pressure is 1,118 psi. The well head assembly is equipped with 2 manual and one emergency shut down valve on the vertical section of the production tree followed by a single manual valve on the horizontal outlet section of the production tree. Also on location is 4 gas processing units along with a production facility tank farm consisting of 4 steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. Proper well identification is posted at the well heads along with hazard and identification on the tank battery. At all critical hazard and muster points of the pad are fire extinguishers and emergency shut down master well control devices. A EQT midstream point is on the right front corner of the well pad.

1866275044 2016-08-15 Casing String Witnessed No Violations I am onsite to witness the conductor installation and cementing operations. Express Energy Services utilized an auger drilling rig to drill a 36 borehole to a depth of 100. Express then installed 100 of 30 .375 wall weld casing. Finally, Express cemented the casing in place with 275 sacks of class A cement with .25 pounds per gallon of flowseal. The cement was verified by scale at 15.6 pounds per gallon. I witnessed 14 barrels of cement circulate to surface. Six inches of cement was left in the cellar to provide a liquid tight seal at the base. While drilling at 42 a moderate flow of fresh water was encountered and remained through the job.
2047906864 2018-01-02 Production Wells No Violations On site inspection as to a response to a report of a 25 gallon fluid release on the EQT midstream production pad that is situated on a site pad that adjoins the main well pad. The pad is situated on the South West side of the Switz 5 well pad and was deplicted on the construction submittal plans in 2015. No product was visible on the earths surface and there was a roll off container on site with material place in it that had been recovered. Addressing the well, the drilling and completion process is now complete and the well is in prodcution along with 3 additional wells on the pad. There is a total of (4) gas processing units on the well pad that the wells produce into. Alos present is a production fluid storage facility consisting of (4) steel concruction 400 barrel capacity production tanks situated inside a polymer lined steel containment dike atrea. Proper identification is posted at multiple locations on site.
