0058396925 2015-05-01 Casing String Witnessed No Violations I am onsite to witness the surface / mine string casing installation and cementing operations. H&P Drilling Rig 531 ran 803 of 18.625 casing with a Davis Lynch hydraulic packer and stage tool. Schlumberger is providing the cementing services. Thirty-six barrels of Surfset class A cement was placed at the base of the casing string. The packer and the stage tool were set and Schlumberger then pumped 235.4 barrels of Surfset class A cement. Circulation could not be established and the fluid level did not reach any higher than fifty feet from surface. The company was instructed to perform a bond log and remedial cementing operations.
1319668849 2018-02-27 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the affected area of the well site construction meets ORC 1509.07 with vegetation established and erosion control in place. The well pad lease road has signage posted and is gated and locked. The well pad has an 8 chain link fence that is also gated and locked with signage posted at the entrance also. There is also a emergency shut down device at the well pad entrance. On the well pad there is 5 active wells present with annular pressure monitoring in place on each well and the well tender reporting no abnormal pressures recorded or encountered. Each well head is configured with a casing flow cross and (2) outlet valves in place however no casing flow lines are attached. Attached to the tubing heads are (2) manual and (2) remotely controlled emergency shut down valves. Each of the 5 wells on location has proper well identification posted. Also on location is a large natural gas compressor followed by a gas processing unit and 5 gas dehydration units, one supporting each well on the pad. At the far left hand side of the well pad is the production fluid storage area consisting of a total of (6) tanks that are steel construction and situated inside a polymer lined containment area.

1426256047 2015-03-13 Casing String Witnessed No Violations The said well was spudded on 03/12/2015 by Precision Rathole Inc with a 120 x 36 conductor borehole being drilled and 120 of 30 conductor being set. Universal well service is on location to cement the conductor using 509 sacks of class A -3% calcium .125 pps flake 100% open hole excess cement.
1430429819 2015-04-30 Casing String Witnessed No Violations . 04/30/2015 -H&P Drilling rig 531 is now on location and has drilled a 24 x 800 surface -mine string borehole and ran 789 of 18.625 87.5# casing with a guide shoe and float collar ran along with a DV tool @ 629 and hydraulic packer @ 632 to isolate the mine void. Schlumberger is on location to cement the casing with a 255 sack Class A mix for the bottom section and a 1105 sack mix for the top section with the Hydraulic packer serving as the zonal isolation tool. The company was advised that the casing did not meet the minimum requirements of 30 below the bottom of the mine void 760 with casing initially to be set at 786.
1430807092 2015-05-04 Casing String Witnessed No Violations Eclipse Resources had HP Flex rig 531 drill a 17.5open hole to depth of 2725. They then ran 13.375casing with 1495 at 68lb/ft. and 1194at 61 lb./ft. All casing is range 3 and J-55 Buttress thread. The casing is centralized as per the casing Talley. Schlumberger is the cementing contractor and will be mixing 2100 sacks of Class A cement at 15.6 lb./gallon with a yield of 1.21 cuft/sack. The schedule is as follows: Rig pumps will break circulation. Then hook up Schlumbergers head and manifold. Pressure test the manifold and lines and pumps only to 3000 psi. Then set pump trips off to 2400 psi. Pump 20 barrels of gel spacer. Then pump 10 barrel of water spacer. Then mix 453 barrels of slurry at 15.6 lb./gallon. Then displace with 399 barrels of water. The mix water temperature is 64 degrees Fahrenheit. The PH is 7.8.
1431368430 2015-05-11 Casing String Witnessed No Violations Status check inspection on the drilling process found H&P rig 531 on location with the 12.250 2nd intermediate well bore total drilling depth reached at 9,612. Wyoming casing service has ran 9,487 of 40# 9.625 in the well bore and is currently conditioning the wellbore in preparation for the cement job to be conducted by Schlumberger.
1432042772 2015-05-19 Casing String Witnessed No Violations Status check inspection on the running of the 5.5 production casing. H&P rig 531 has drilled a production borehole to a depth of 19,208 with a true vertical depth of 10,519 in the directional azimuth of 149.23 degrees with a borehole inclination of 91.71 degrees. The company is currently running casing at a depth of 13,786 averaging 1,300 feet per hour. A casing tally was received showing 19,197 of 23# HCP-110 casing to be installed with a total of 4 Nine Energy Flow guns set in the casing string from 19,059 to 18,896.
1441896098 2015-09-10 Completion Testing No Violations Follow up inspection to a report received on 09/07/2015 at 14:00 pm by Bruce Carpenter with Eclipse who indicated that Halliburton Stimulation crew was on location and during the initial stimulation process had a failure of a 4 diameter by 4 long pressure treatment iron pipe in which caused approximately 200 gallons of a mixture of produced water and fresh water to be released onto the well pad inside the working containment area. On this day I visited the well site and received a copy of the treatment chart from the completion stage during the failure which indicated that on 09/06/2015 at 17:58 pm the failure occurred during stage one of 40 stages total to be completed, the stage chart indicates the operation was down for 10 minutes and no injuries were reported with the product being recovered off the containment by the means of a vacuum truck. Addressing the current well completion status on this day Halliburton is stimulating stage 4 with the backside annular pressure being monitored electronically. Nine Energy wireline is on location and has perforated the stage prior to stimulation with a solid completion plug set at 18,188 and 40 perforations being shot in 5 clusters between the depths of 18,165 and 17,979.

Addressing the well head - The drilling and completion process is complete and a production well head tree is in place on the well, on location on this day is Tetra Services providing flow back testing operations. The following permanent production equipment is in place downstream from the well - A sand trap along with a inline heater and fluid separator the gas then flows into the main sales line with 2 -Dehydration units in place on location, the production fluid flows into a tank battery consisting of 6 tanks of 400 barrel fluid capacity which is present inside a polymer lined steel containment dike area which is located at the rear of the well pad.
