0298932387 2015-06-12 Casing String Witnessed No Violations 06/11/2015- H&P Drilling rig # 531 is now on location and has drilled a 24 surface-mine string borehole to a depth of 862 with the mine void being encountered at a depth of 751 to 762 with 795 of 18.625 87.5# J-55 casing to be ran and a 2 stage tool at 617 along with a hydraulic set formation packer set below the 2 stage tool at 621, 2 cement baskets were set at 577 and 637. 06/12/2015- Schlumberger cement service is on location to cement the casing in a 2 part procedure with the lower section cemented using 140 sacks of Class A (Surface set ) cement, upon the landing of the plug the pressure was increased until the hydraulic operated inflatable packer set and was confirmed to be holding, the 2 stage DV tool was then opened and circulation established then allowing the 2nd cement stage to be implemented using 1420 sacks of Class A (Surface Set) cement. During the 2nd stage displacement no cement returns were witnessed to surface, good lift pressure and circulation was witnessed. The cement top was tagged on the annular side at 20 from surface after the job, the company then topped the annular side off to ground level with 15 barrels of class A cement. The casing string will be bond logged.
1258155975 2015-06-21 Casing String Witnessed No Violations On this day I was notified by the Company Representative on site at 12:59 pm that the 9.625casing had became stuck while running it into the well bore with a reported 12.250 well bore drilled to a depth of 9,464, the 9.625 casing shoe is at 9,177 and the casing float collar is at 9,086. The casing was free pointed and perforated at a depth of 8,856 to 8,864 shooting one perforation every 12. Circulation was then established and the cement job administered by Schlumberger. All records received and my Regional Supervisor was updated on the status on this day.
1426079278 2015-03-11 Casing String Witnessed No Violations Status check inspection to witness the conductor cement job. On this day the following was observed-Precision Rathole Inc has drilled a 36 x 120 conductor borehole and ran 120 of 30 .375 wall pipe with Universal well service on location to cement the casing using 360 sacks of class A 3% calcium .125 pps flake 50% open hole excess cement with the cement displaced to 100 using 83 bbl of fresh water with no cement returns witnessed to surface, the hole was topped off with 50 bbl of class A cement along with 1/2 load of Redi mix 9 bag grout mix on this day. The cement mix water was tested for PH & Chlorides.

1434383330 2015-06-15 Casing String Witnessed No Violations Status check inspection on the cementing of the 13.375 1st intermediate well bore casing string found H&P Drilling rig 531 has drilled a 17.5 1st intermediate well bore to a depth of 2,663 and ran a total of 2,658 of 13.375 casing with Schlumberger cement service on location to cement the casing using 1950 sacks of class A cement with 12 barrels of cement returns to surface and the casing pressure tested to 1,590 psi upon the landing of the plug.
1435695090 2015-06-30 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found H&P Drilling rig 531 on location with the 8.750 horizontal well bore section total drilling depth recently reached at a measured depth of 19,976 in the directional Azimuth of 157.33 degrees with the drilling rig currently running the 5.5 production casing at a depth of 7,876.
1439313160 2015-08-11 Completion Testing No Violations Status check inspection on the well completion process found Deepwell Services Workover rig on location. The company has completed the 5.5 production casing post stimulation casing toe preparation. HPC well control company is on location and providing well head surface control. It was observed that the service rig was pulling a wet string and had not evacuated the brine water from the workover tubing string allowing the product to be discharged onto the working area. The company men were advised that the discharge method was not acceptable and the tubing should be evacuated with fresh water prior to pulling the workover string. The product was tested with Quantab field strip test with readings exceeding the high range strip tests at 6,171 parts per million and exceeding the low range strip tests at 611 parts per million. Eclipse HSE supervisor Rocky King was notified that the workers were allowing the product to be released on the containment area with no perimeter berms completely surrounding the working area and the product was not going into the cellar.
1448371615 2015-11-24 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area indicated as affected by the well pad construction has been restored with proper erosion control and vegetation in place. Addressing the well head - The drilling and completion process is complete and a production well head tree is in place on the well, on location on this day is Tetra Services providing flow back testing operations. The following permanent production equipment is in place downstream from the well - A sand trap along with a inline heater and fluid separator the gas then flows into the main sales line with 2 -Dehydration units in place on location, the production fluid flows into a tank battery consisting of 6 tanks of 400 barrel fluid capacity which is present inside a polymer lined steel containment dike area which is located at the rear of the well pad.
1519744048 2018-02-27 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the affected area of the well site construction meets ORC 1509.07 with vegetation established and erosion control in place. The well pad lease road has signage posted and is gated and locked. The well pad has an 8 chain link fence that is also gated and locked with signage posted at the entrance also. There is also a emergency shut down device at the well pad entrance. On the well pad there is 5 active wells present with annular pressure monitoring in place on each well and the well tender reporting no abnormal pressures recorded or encountered. Each well head is configured with a casing flow cross and (2) outlet valves in place however no casing flow lines are attached. Attached to the tubing heads are (2) manual and (2) remotely controlled emergency shut down valves. Each of the 5 wells on location has proper well identification posted. Also on location is a large natural gas compressor followed by a gas processing unit and 5 gas dehydration units, one supporting each well on the pad. At the far left hand side of the well pad is the production fluid storage area consisting of a total of (6) tanks that are steel construction and situated inside a polymer lined containment area.
