0898352956 | 2018-02-27 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the affected area of the well site construction meets ORC 1509.07 with vegetation established and erosion control in place. The well pad lease road has signage posted and is gated and locked. The well pad has an 8 chain link fence that is also gated and locked with signage posted at the entrance also. There is also a emergency shut down device at the well pad entrance. On the well pad there is 5 active wells present with annular pressure monitoring in place on each well and the well tender reporting no abnormal pressures recorded or encountered. Each well head is configured with a casing flow cross and (2) outlet valves in place however no casing flow lines are attached. Attached to the tubing heads are (2) manual and (2) remotely controlled emergency shut down valves. Each of the 5 wells on location has proper well identification posted. Also on location is a large natural gas compressor followed by a gas processing unit and 5 gas dehydration units, one supporting each well on the pad. At the far left hand side of the well pad is the production fluid storage area consisting of a total of (6) tanks that are steel construction and situated inside a polymer lined containment area. |
0965457908 | 2015-03-11 | Casing String Witnessed | No Violations | Status check inspection to witness the conductor cement job. On this day the following was observed-Precision Rathole Inc has drilled a 36 x 120 conductor borehole and ran 120 of 30 .375 wall pipe with Universal well service on location to cement the casing using 360 sacks of class A 3% calcium .125 pps flake 50% open hole excess cement with the cement displaced to 100 using 83 bbl of fresh water with no cement returns witnessed to surface on the initial job, the hole was topped off with 50 bbl of class A cement along with 1/2 load of Redi mix 9 bag grout mix. The cement mix water was tested for PH & Chlorides. |
1138154085 | 2015-11-24 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the area indicated as affected by the well pad construction has been restored with proper erosion control and vegetation in place. Addressing the well head - The drilling and completion process is complete and a production well head tree is in place on the well, on location on this day is Tetra Services providing flow back testing operations. The following permanent production equipment is in place downstream from the well - A sand trap along with a inline heater and fluid separator the gas then flows into the main sales line with 2 -Dehydration units in place on location, the production fluid flows into a tank battery consisting of 6 tanks of 400 barrel fluid capacity which is present inside a polymer lined steel containment dike area which is located at the rear of the well pad. |
1428121321 | 2015-04-04 | Drill / Deepen / Reopen | No Violations | H&P Drilling rig 531 is now on location and has drilled a Mine Void-Surface hole to a depth of 810 with the mine void encountered @ 756 to 760 with circulation lost to surface from 756 to TD @ 810. The surface casing and mine string were set as one string with 794 of 18.625 casing set with a float shoe and collar ran to isolate the mine void. The 2 stage hydraulic tool and Hydraulic packer set will isolate the upper casing section above the mine void. Schlumberger Cement service is on location to cement the casing using 205 sacks of class A on the lower section and 1090 sacks of class A on the upper section to be cemented. |
1429635714 | 2015-04-21 | Plug / Plug Back | No Violations | The said well was drilled vertically to a measured depth of 10,730 and plugged back to 9,586 on 04/19/2015. With the current drilling status on 04/21/2015 found H&P Drilling rig 531 on location drilling the 8.750 production borehole at a depth of 9,864 with the company deviating out of the cement and into 100% formation. |

1435438882 | 2015-04-15 | Drill / Deepen / Reopen | No Violations | Status check on site inspection found the company has recently completed the drilling of the 12.250 2nd intermediate well bore to a depth of 8,976. Initially when the drill string was pulled out of the hole it was discovered that one of the cones on the tri-cone drill bit had fallen off and the company had requested my presence on site to witness the setting of a deviation plug. Upon arrival to witness the plug being set I was advised that they were able to recover the lost iron in the hole and was now going to run the 9.625 casing and cement as planned. |
1584271987 | 2018-02-27 | Final Restoration | No Violations | The said well was permitted as a strata graphical test and was plugged back and the permit number for the existing well is now 3411124570-0100. |
1619194372 | 2015-04-07 | Casing String Witnessed | No Violations | H P Flex rig 531 had ran 13.375casing in with casing crew. There was 1283of 13.375casing @61lb/ft of j-55Butress thread. There was 1260 of 68 lb/ft of j-55 Butress thread. The total pipe was at 2547. It was placed in a 17.5open hole at a depth of 2595. The casing shoe was at 2543. Float collar was ran. There were 21 centrilizers and no baskets ran. Schlumberger was the cementing contractor and ran 1755 sacks of Class A cement. The yield was 1.21 cuft/sack and the water requirement was at 5.2 gal /sack. After the rig washed in 30 of pipe and tied Schlumberger on they tested lines and then pumped 20 barrels of gel preflush and then 5 barrel water spacer. Gel or cement did not come back to surface. Circulation was not lost so not enough excess was ran. Company only ran 25%excess on the open hole section. |
