0638303953 2016-08-22 Casing String Witnessed No Violations On site inspection to witness the 18.625 mine string annular side cement grout job. Allied Cement service was on location and pumped 92 sacks of class A cement (20 bbl) through a 1 grout string set to 300; the tubing was pulled to 200 and cleared and operations were then shut down to allow the cement to cure for 2.5 hours. A 2nd 31 barrel class A 3% calcium cement mix was spotted and will be allowed to cure before running the grout string back in and attempting to tag the cement top and finishing the top out job off.

0930048350 2019-05-02 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (5) producing wells , (4) gas processing units with sand traps, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance, tanks, and wellheads. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 457 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 28 psi. The production records are current.
1065043283 2017-08-16 Production Wells No Violations The purpose of my inspection was a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure gauges present read 2613 pounds per square inch on the tubing, 2953 pounds per square inch on the 5.5 inch diameter casing, 135 pounds per square inch on the 13.375 inch diameter first intermediate casing, 855 pounds per square inch on the 9.625 inch diameter second intermediate casing. The production equipment consisted of four 400 barrel tanks, five sand traps, and five gas production units. Identification was posted at the wellhead.
1220592323 2015-06-03 Casing String Witnessed No Violations The said well by Express Energys top hole rig with a 100 x 36 conductor borehole being drilled and 100 of 30 conductor being set. Express Energy cement service cemented the casing using 275 sacks of class A -2% calcium .50 pound per sack loss circulation material 50% open hole excess cement with good cement returns witnessed to surface and the cement mix water tested for chlorides and PH.
1409546625 2016-08-18 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found Patterson 801 on location with the 24 mine string borehole recently drilled to 768 and a coal seam encountered at 688 to 693, the company men on site called on the evening of 08/17/2016 and indicated they did not encounter a mine void, the rig was directed to load the hole with fresh water and if the hole loaded no formation packer or DV tool would be required. The well bore broke down under hydrostatic pressure and did not load. The Company will run the formation packer and DV tool above the mine void. Gas was also encountered in the mine void. At the time of my inspection at 12:00 pm the rig was waiting on additional joints of 18.625 casing and indicated they would not be ready for at least 6 hours.
1471732504 2016-08-20 Casing String Witnessed No Violations On site inspection to witness the Mine String -surface casing. Patterson rig 801 is on location and has drilled a 24 borehole to 770 with a mine void encountered at 688 to 693. The rig then ran 735 of 18.625 87.5 lb/ft J-55 casing with a guide shoe ran on bottom and a float collar set at 605; a swell packer was set at 563 with a 2 stage cement tool directly above. The bottom section cement job consisted of 280 sacks of 15.6 lb/gl class A cement with the tool being deployed and circulation was not established to surface before initiating the 2nd stage of the cementing process consisting of 1059 sacks of class A cement. Cement returns were not witnessed to surface on the 2nd stage. The company has indicated the formation packer deployed. I have instructed the company to run a 1 grout string and properly cement the 18.625 annular area accoringly. Only the bottom stage was witness due to the extended period of time on location with contact made with my supervisor Brent Bear before departing location after 8 hours on location.
1478783668 2016-11-09 Drill / Deepen / Reopen No Violations During my inspection Allied Oilfield Services was rigging down from cementing the 9.625 inch diameter Intermediate casing. The 9.625 inch diameter casing was set at a depth of 9306 feet.
1528140710 2018-06-04 Work Over Reconditioning No Violations Status check inspection found a workover rig on the well owned by Deep Well Services, there is a workover blow out preventer on the well. The rig is fishing a lost 2.375 workover string of coil tubing lost in the well bore with the top of the tubing located at 11,018. The company man on site indicated they have washed over the lost string down to 11,480. The company is maintaining control over the 3,143 psi surface pressure using barrel in barrel out circulation using 12.5 pound per gallon production fluid. The 5.5 diameter production casing was set to 16,740 and the top perforation is at 10,848. The company man on site indicated he has a 45 day allowed time frame to attempt to recover the tubing.
1529514775 2018-06-20 Completion Testing No Violations Status check inspection found a service rig owned by Well Service Group rigged up on the hole and fishing a lost section of 2.375 coil tubing. The tubing top is at 12,001 measured depth. The rig has been fishing on the tubing for 34 days. The top perforation in the 5.5 diameter production casing is at 10,848 and the well bore true vertical depth is 10,400.
1533578388 2018-08-06 Work Over Reconditioning No Violations Status check inspection on the efforts to recover a lost 2.375 coil tubing string inside the 5.5diameter production casing string. The top of the tubing is currently at a measured depth of 14,672 . A service rig owned by Key Energy is rigged up on the well and is running in the hole with a wash over string consisting of 18 joints of 3.5 diameter 2,920 inside diameter high drill wash over pipe, the joint length is 32 average and the workover string is 2.875 PH6 tubing. The well bore true vertical depth is 10,400. The well bore is static with 9.6 pound per gallon brine. There is a BOP stack and snubbing unit on the well. The rig and support equipment is situated on polymer liner. This is a multi well pad.
1610041582 2016-10-31 Casing String Witnessed No Violations The purpose of my inspection is to witness the 18.625 inch diameter surface casing being cemented in a 24 inch diameter hole from a depth of 785 feet to 100 feet and a 30 inch diameter hole from 100 feet to surface. The casing shoe is at 745 feet and the Differential Valve Tool (DV Tool) is at 438 feet. Allied Oilfield Services are on location to complete the 2 stage cement job. During my inspection I attended the Pre-job safety meeting and then stood by the Allied company truck watching the computer that reads the density and the pressures of the pump truck. 240 barrels of H20 were used to break circulation, 10 barrels of gel and 10 barrels of water were pumped ahead of the 47.5 barrels of cement at 15.6 pounds per gallon. A sample of the cement was pulled and weighed in the portable field scales and the cement weighed 15.6 pounds per gallon. The pump truck shut down and the plug was released. The packer was set at 440 feet. The DV tool was opened and stage two began. 140 barrels were pumped to break circulation. 25 barrels of gel, 10 barrels of H20, and 150 barrels of cement at 15.6 pounds per gallon were pumped. A sample of the cement was pulled and weighed in the portable field scales and the cement weighed 15.6 pounds per gallon. The pump truck shut down to release the plug. 134 barrels were used for displacement and I witnessed roughly 45 barrels of cement returned to surface to the half round tanks.
