0396933913 | 2015-11-10 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the area affected by the well pad construction has been restored with proper erosion control and vegetation in place. There is a total of 5 wells spudded on the pad, none of which are completed at this time. No production equipment is present on the well pad. |

1433364548 | 2015-06-22 | Casing String Witnessed | No Violations | The said well by Express Energys top hole rig with a 100 x 36 conductor borehole being drilled and 100 of 30 conductor being set. Express Energy cement service cemented the casing using 275 sacks of class A -2% calcium .50 pound per sack loss circulation material 50% open hole excess cement with good cement returns witnessed to surface and the cement mix water tested for chlorides and PH. |
1474148818 | 2016-09-17 | Casing String Witnessed | No Violations | On site inspection to witness the 18.625surface-mine string combination with Patterson Drilling rig # 801 on location and a 24 borehole drilled to 770 with the mine seam encountered at 690 to 697, a 18.625 87.5 lb/ft J-55 Buttress thread casing string was ran to a depth of 742 with a hydraulic operated inflatable packer set at 456 and a DV tool set above the packer at 447. Allied Cement service was on location to cement the casing with the bottom section between the casing shoe and mine void cemented by first pumping 50 barrels of 6% gel followed by 260 sacks of class A-2% calcium cement that was displaced with 226 barrels of fresh water; the swell packer was the inflated by increasing the internal casing pressure to 1,047 psi and then the DV tool was then opened with circulation established to surface before beginning the upper section cement stage by first pumping 50 barrels of 6% gel spacer followed by 1079 sacks or 230 barrels of class A-2% calcium cement with the cement displaced with 196 barrels of fresh water and casing integrity verified by pressure during the landing of both cement plugs on both cement sections with cement returns to surface on the last stage. |
1660564643 | 2017-08-16 | Production Wells | No Violations | The purpose of my inspection was a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure gauges present read 5920 pounds per square inch on the tubing, 6610 pounds per square inch on the 5.5 inch diameter casing, 590 pounds per square inch on the 13.375 inch diameter first intermediate casing, 386 pounds per square inch on the 9.625 inch diameter second intermediate casing. The production equipment consisted of four 400 barrel tanks, five sand traps, and five gas production units. Identification was posted at the wellhead. |
2000043505 | 2019-05-02 | Production Wells | No Violations | On this day, I found the pad gated with signage at the entrance. There are (5) producing wells , (4) gas processing units with sand traps, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance, tanks, and wellheads. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 460 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 57 psi. The production records are current. |
2102631889 | 2016-09-20 | Drill / Deepen / Reopen | No Violations | Status check inspection on the current well bore drilling process accompanied by ODNR Inspector Brian Sefcik found Patterson Drilling rig 801 on location and the 17.5 1st intermediate well bore total drilling depth reached at 2,580 and the company currently running the 13.375 1st intermediate casing string to be set at 2,529 measured true vertical depth. Allied Cement Service is on location to cement the casing with a lead and tail 30% excess class A cement mix. The Berea top was marked at 2,310 by the mud loggers. |
